The company provides and assurance plan for the surgery and the charge listed on the bill is $per eye They have an opt out form but when I told them I wanted to opt out they said that the deduct is only $per eye and not the $listed on the itemized quote I don't understand how an itemized statement does not reflect the amount deducted once you opt out of the assurance plan which is optional This seems like a scam Product_Or_Service: [redacted] Plan Order_Number: ORDXXX-XXXXXX

I paid for services for Lasik 11/3/that was cancelledI have been reaching out to get a refund for my money but I am getting the run around service date 11/3/17- was advised I would have the funds in weeksI needs the funds reversed to my flexible spending and care credit cards as I need to use my flexible spending by the end of the yearIn addition, I applied for care credit to supplement the cost for the procedure and I now have a statement due 12/If I don't pay that I will be charged late fees and reported to credit bureausI need the funds returned to that car as wellI am unable to make doctor appointments as the money isn't back on my flexible spending card

I gave this company a check for $in JanuaryThey told me someone would call me, didn't After repeated calls now asking for refund, nothing I went to Lasik Vision Institute in January to see if I was a candidate for lasik surgeryBecause of cataracts I am notThey convinced me I should have cataract surgery and I paid them $They told me someone would call me in or days to schedule the surgeryAfter two weeks I bagan to callNo one ever answered the phone at the Tampa officeI finally got a manager at their corporate office to have someone call meShe stated she had called and missed me several timesHowever, my phone showed no record that that had happenedI gave her dates I could have the surgeryShe said she would call right backShe never didAfter multiple complaints to their corporate office the Tampa manager finally called to set a dateBy this time I was no longer free on that dateThis was in late February or early MarchI asked for a refundI

Violation of US canned spam laws by mass spamming my email account
Lasik Vision is in violation of the US canned spam laws by spamming my email accountI have not opted in as required by the US canned spams laws and have no existing business relationship and have no way given them permission to send electronic spam (advertising) to meTheir distribution into my email account is a purposeful and willful violation of US canned spam laws

Lasik Vision Institute guaranteed that a specific doctor that was requested would be present to conduct the surgeryThis was in writing on all documentationDuring a pvisit the day before the surgery no notification was given that the preferred doctor was not inThe day of Surgery, Rae, who guaranteed this doctor, never mentioned the doctor would not be inAnother doctor was in that dayDuring the hour wait, which was supposed to 1-hours given the pre=op visit the previous day, Rae avoided the entire issue and hid in her officeThe ignorant manager mismanaged the files of all of the patients that day and the doctor became irateHe publicly scolded the manager for her incompetence, after hurting me during surgery due to his impatience and aggravation with the staff

On 4/1/I decided to have vision correction surgeryI was told by the doctor that I should get something called mono vision surgery, I agreed to this procedureOn the day of surgery the surgeon talked me into a different procedure telling me that it would be enough of a correctionIt did nothing to help my visionMy eye burns and sometimes is hard to open in the mornings, it feels like I have to pry it openI wrote a letter in October of asking to settle the matter with themMy letter was ignoredI resent the same letter it was again ignoredI would like my money $1,refundedI know that my eye will never be the sameI understand from other eye care professionals that there is no going back to fix the problemI feel that my vision was impaired instead of improved by this procedureI paid an extra amount to guarantee a lifetime benefitThis company does not care about their patients only the money they make

I received services from the Lasik Vision Institute on 9/8/After receiving services I started to get unexpected bills from their financing company (HealthCare Finance Direct)After requesting copies of the loan documentation - I realized I did not sign or authorize *** *** (Office Manager) to sign these papers on my behalfI spoke with not only *** who is employed with The Lasik Vision Institute but also the actual finance companyNeither of the companies will provide a corporate # for someone higher than *** that I can speak with regarding the fraud and forgery of these documents by *** ***They only information they will provide is stating "the documents are valid and were compared to the consent for treatment that I signedLooking at all the documents and comparing it is very clear with a naked eye that the signatures and initials are not the sameIn fact the writing matches how *** *** signs and initials her name
