Revdex.com called business to follow up on this complaint and spoke to [redacted], manager, who said the consumer has been taken care of. She doesn't owe anything, has a zero balance and is happy with the resolution.

This matter was addressed already.  Unfortunately Mr. [redacted] does not have a current phone number on file so an email was sent to [redacted]  stating:     I tried to return your call and the number we had on file was not yours. ...

Dawn is cancelled effective today and a refund for $21.50 will be returned.  Your membership was terminated with no more billing as requested.  As stated on the cancellation form there must be a separate cancellation form for each member.  Unfortunately the billing department didn't know that she was to be cancelled.  If you have any further questions please contact the club.  This was a courtesy and as stated on every membership agreement that a written 30 day notice must be given by sending a certified letter or stopping at the club.
