Revdex.com: I have reviewed the response made by the business in reference to complaint ID [redacted] , and have determined that this proposed action would not resolve my complaint For your reference, details of the offer I reviewed appear below Regards, [redacted] My request to bill Medicare for only part of the surgery which you promised to help THE MAIN OJECYIONFROM MEDICARE WASMEC TRYING TO SAY THE SURGERY WAS DONE ON 9/2/but the office keeps sending an incorrect date of 8/26/and I was working that day so it is difficult to know where they got this date I completely did all the paperwork for the doctor and all he had to do was read it sign and send it Back to MEDICARE I HAVE SPENT HOURS TRYING TO CONVINCE MEDICARE THAT I AM NOT LYING ABOUT THE SURGERY DATE ON 9/2/which I remember because it was hours late as far as saying I never asked you for help maybe you should check with your staff and note I many times I call as far as my PAST PPO that was really out of my control since the MRI COMPANY did not send the MRI TRULY SORRY THIS HAD TO BECOME THIS ISSUE OVER AN INCORRECT DATE X

To Whom It May Concern:This is a reply to your request for information regarding the complaint submitted by the above named customer.We are working diligently to provide the customer with documentations that she requests. The challenge in providing her with this information stems from the fact that...

the surgeon, Dr. [redacted] does very little work with Medicare patients. The surgical facility, mosaic surgery center, where the customer's procedure was performed is not contracted with Medicare. Therefore, the facility has never submitted any bills/claims for operating room fee and anesthesia fee to Medicare. In order to comply with her request, we are trying to create a new bill/claim from scratch. I also do not have the requested certificate a medical necessity since one was never submitted to Medicare.Prior to her surgery, we tried diligently to work with the patient in getting her preauthorization approved through her PPO insurance. But after months of delay and cancellations, patient suddenly decided schedule her surgery and she wanted to pay for the surgery out of pocket. We were under the impression that she did not want to bill through her insurance so no claims were created or submitted.Due to HIPAA regulation, I will send the requested documents directly to my patient but no other. I can be contacted at my office should you need to discuss this complaint further.
