Re: ID [redacted]
Dear Ms. [redacted]
This letter is in response to ID [redacted]. Patient states that our procedures are not safe, Fraxel and Artefill are both FDA approved (please see enclosed brochures). Dr. [redacted] is a certified trainer for Artefill, he has a Diamond Elite Status for injecting Artefill. Furthermore, he is certified to train other Doctors across the Country.
Dr. [redacted] is the only one in this office that injects fillers and botox injections. Our Registered nurse does the Fraxel and IPL treatments. Also, in her letter she states that [redacted] was the salesperson which is wrong, I'm the Administrator. I never sold her any procedures or products. She was seen by our trained personnel for consultations and product knowledge. I only saw her when she came to complain about her husband being upset at her and about her procedures which I always helped her and would send her to Dr. [redacted] right away.
Patient first came to our office on June 8, 2014, she wanted fraxel to help with her acne scars. She was given a consultation on the product, which she purchased on her own free will and signed a consent form for fraxel laser before she had her treatment. She also purchased Obagi products to help with her treatment. She applied for Care Credit to finance her treatments. The treatment was done by our Registered Nurse on June 15, 2012. She was given a free of charge microdermabration treatment for her purchase and had treatment done on June 29, 2012. She had her 2nd fraxel treatment on July 13, 2012, on that same date she wanted a consultation on Artefill filler. She stated she wanted her temples filled and her nose filled with fillers (she had a bump on her nose and wanted it filled). She signed them consent form and Dr. [redacted] injected her fillers.
Our office staffed called her the next day to make sure she was okay after her treatment which is standard at our practice, we always follow up with the patient to make sure they are doing well. She did not answer, message was left. On July 23, 2012 our office staff contacted her again to make sure she was satisfied with her treatment . Message was left on her phone and a Thank You card with a free microdermabration treatment was sent to her via US mail. On August 17, 2012 she came and had her free microdermabration treatment. On that day, she again stated that her husband was very upset at her because she had spent so much money and she was very worried. She then complained that she was not satisfied with her Artefill filler. She said she had spend a lot of money and that she wanted more filler on her temples because according to her she wanted to look 10 years younger and did not see any results. Dr. [redacted] saw her, heard her complain and gave her a free Artefill syringed valued at $900.00 on her temples. She left very happy. On September 28, 2012, she came to the office and complained this time about the Artefill filler given on her nose. She stated that she was unhappy with her nose and wanted more effect. Dr. [redacted] injected her with Radiesse (a filler)-valued at $550.00 free of charge so she would be happy and see more results. He also gave her 40 units of Botox -value $400.00 free of charge. She also complained about her Fraxel treatment. She has significant Rosacea, so Dr. [redacted] gave her IPL treatment to help her with her rosacea valued at $250.00 each. On October 23, 2012, she had a follow up with Dr. [redacted], patient still complains of her nose, Dr. [redacted] gives her another Radiesse -valued at $550.00 free of charge. He gave her a total of 5 IPL treatments $1,250.00. Her treatments were done on 9/28/12, 10/26/12, 12/7/12, 1/18/13 and 4/26/13. She was given a total of $3,650.00 worth of filler and treatments for free.
On September 10, 2012, we received a billing dispute with GE Capital (Care Credit) by patient ID [redacted]. Her dispute reason was: Service Quality. It stated "Cardholder states she had a nose job done and she is not satisfied. She wanted to see before and after pictures because she says there is no change on her nose and all they tell her is that person responsible for that is not available". Again, she had something different to complain about. We responded to this dispute (please see attached letter sent to GE Capital along with their response-Dispute Resolved in our favor).
As you can see, Dr. [redacted] tried very hard to have this patient satisfied with her treatments but it was impossible. She continued to take advantage of her free IPL treatments even after her dispute and we continue to honor want Dr. [redacted] gave her (Her IPL treatments for free). Even after her dispute, if she felt we were unsafe why did she continue coming to our office? In conclusion we have tried unsuccessfully to work with her. Patient keeps changing her story. She has unrealistic expectations for a non-surgical procedure.
Attached, is a copy of the signed Care Credit contract and application, a signed sales draft, and the dispute letter and resolution from GE Capital and copies of all services provided with patients signature and dates, brochures on FDA approved Artefill and Fraxel treatments. Should you have any questions, please feel free to contact me at the above phone number or contact Dr. [redacted] on his cell phone.
