Bureau We have reviewed the dispute made by patient. We never received a complaint regarding tooth #3 which was cemented on 10-24-2017. We have attached all document's requested and confirmed all treatment has been completed. In regard's to teeth #8 and #9 doctor completed a temporary filling with no **arantee patient's symptoms would subside since patient needs Rct's on teeth #8 and #9 which is diagnosed on 11,-25-2017_ Teeth #8 and #9 is not part of treatment plan dated 8-14-2017, in which patient paid $2,520.00 which work has been rendered. Please be advised we informed patient of new diagnoses on teeth #8 and #9 and there is no relation to tooth #3, We also informed patient we didn't charge him for filling's completed on teeth #8 and #9, and if he did not agree with doctor **'s diagnose's on 11-25-2017 he may have a 2nd opinion. All treatment has been rendered to patient, attached document's. Total treatment paid and rendered $2,520.00. New diagnoses total treatment $5,400.00, has not been rendered and patient has not paid. Please note we have attached a dispute patient made to CareCredit and has been resolved with NO CHARGEBACK indicated.
