Let it be known that my physican had reffered me to this office to have the X-ray performedThe office appointment was made between my doctor and California imagingTherefore my correct information had been allready forwarded from the doctors office to California imagingMisspelling my name goes to show that the error was made on their partThe address they pulled on Magill was over years oldAs a child, I once went to California imaging for an injury X-rayThat's where someone from CI obviously made an error, saw my name, right clicked on a screen and pulled my previous addressMy doctors office had all the correct info as they had referred me to other labs and therapists with no similar address issuesWhy would you have trouble finding me when all you had to do is call my attending physican to correct the issue and find out about my whereabouts?Please correct the mistake which has been made by California Imaging and is reflecting negatively on my credit report

After fully reviewing [redacted]'s account Y have found the following:
A total of -Four statements were sent on the account dated 6/4/2015, 7/02/2015, 7/30/2015 & 8/27/2015. Statement dated 7/30/201.5 was a final notice that states...

the following! THIS IS YOUR FINAL NOTICE! We have either billed your insurance and this is your balance, or we do not have insurance information on file. We understand that many of our patients experience financial difficulties. If this is the case and you ore unable to pay your bill in full, contact our billing office at (XXX) XXX-XXXX, Be advised that payments made that ore less than the total balance due on your most recent statement, will not hold this account from going to the collection agency without a formal payment plan agreement in place. If you have already sent payment in full, thank you. Otherwise, we look forward to receiving your payment within ten days of this letter. Your prompt attention is appreciated. This is your final notice before assigning your account to a collection agency. We would prefer not to go this direction and would be more than happy to work with you in any fair, equitable way possible but we must hear from you immediately. Mrs. [redacted] was making partial payments on the account but did not contact our office to make a formal payment plan agreement to hold the account from going to collections as stated in the notice. Statement dated 8/27/2015 was a collection notice that states the following: COLLECTION NOTICE! This is our final effort to have you pay your past due account. Your account will be turned over to our collection agency and/or the credit bureau or suit filed in small claims court unless we receive payment within the next 15 days. Please understand that not bringing your account up to date could adversely affect your credit rating_ Respond to this collection notice today. The account did get turned over to collections on 8/31/2015. However, if payment is made within the first 30 days nothing is reported to the credit bureaus and it will not show on the Mrs. [redacted]'s credit report allowing even more than 15 days for payment: to be made. Mrs [redacted]'s account was paid in full with her final payment of $53.64 and the transferred credit from her husband's account of $29.74. This was done within the 30 day time frame so the account was canceled from the collection agency and no credit reporting took place. There is no reference as to which Fair Business Practice code Mrs. [redacted] believes we are in violation of so T am not able to comment on that. Regarding the credit on Mr [redacted]'s account of $29.74 this has since been applied to Mrs [redacted]'s account as she requested. We are not able to move money from one patient's account to another without authorization from the patient. The credit on Mr [redacted]'s account had not yet been identified for a refund to be processed.
Desired resolution has been met with nothing being reported on tree consumer's credit report and we continue to abide by fair business practices.

First of all I am not sure why half of that response was even written since the bill has been paid which finally is referred to in the last half. You are correct in that I made half payment of $200.00 with the first bill received. I then made an additional payment of $200.00 on the next bill which left a remainder of $12.64 which should have been taken care of by the credit of $29.74. We understand that this was in my husbands name but our entire family is billed under the same insurance number. We find it very hard to believe you couldn't have seen this. Its also very frustration for consumers that when you want to get paid you take all the actions necessary but when someone is owed a credit it sits there for 4 months. How is that fair or good business? In response to the "Collections Notice", it states ....Your account will be turned over to our collection agency and/or the credit bureau or suit filed in small claims court unless we receive payment within the next 15 days. You admitted in you response that this indeed was done prior to that 15 day period. The notice was dated on 8-27-15 and you sent it to collections on 8-31-15. Obviously that is not 15 days. On your own admission you have violated fair business practices by doing this.

Consumer's desired resolution has been met. Nothing has been reporting on their consumer credit report and fair business practices have been followed. Fair business practices include the consumer receiving two statements (6/4/2015 & 7/2/2015), a final notice dated 7/30/2015 stating their account would be sent to collections if payment was not received within 10 days of the letter and the Collection Notice dated 8/27/2015. Thanks to the consumer's feedback, we have altered the Collection Notice language to no longer state there is an additional 15 day waiting period.
