The above patient came in on 1/**/2014. After consulting with Dr. B[redacted], the entire procedure was discussed and agreed upon. To be in compliance with HIPPA and patient privacy we are unable to give more details about the services. The procedures were performed and submitted to the patient's...

insurance company. They are documented in the operative report and letter to the referring doctor. The Insurance denied one of the procedures because it was not within her coverage. After appealing with the insurance company and still being denied, we reduced the monies owed from $750 to $500. This was discussed and agreed upon with our office manager and the patient. Statements were sent to the patient and after multiple attempts to collect payment, the patient was sent to collections.
