Ms [redacted] was seen as a new patient by Dr [redacted] at our clinic for her health condition on 4/25/During this appointment Dr [redacted] advised her to have lab work and scans to help evaluate her conditionHe advised her to follow up with an appointment in 2-weeks with test results.We had been waiting for her to follow up on these recommendationsTo date we have not received her lab resultsIn the meanwhile our staff received a call from Ms [redacted] enquiring whether we had her FMLA forms readyOur staff responded by informing her that we are still waiting on her test results for more information pertaining to her health condition and that there is an administrative/service fee of $that is applied for filling out and preparing her formThis is our office policy and Ms [redacted] has signed our financial policy forms at registration which indicates form fees of $When our staff tried to explain all of this to Ms [redacted] she became argumentative and threatened our staff that she would write a bad review on the webShe also asked our office staff if we want to lose her as a patient for $Our staff explained to her that for proper filling of the form she is to have the labs done and also that we are required to follow our office policy; she did not agree and continued to dispute thisWhen she asked for Dr [redacted] to call her about this, she was informed her that the doctor would not be available to discuss with her with regards to fee for forms or services which is typically handled by our office staff.Under these circumstances we had to send her a letter of termination from our practiceOur office called her another time to see if she would be willing to come in for a follow up visit with Dr [redacted] to go over her radiology results but she informed our office staff that she would be going back to her specialist from whom she was transferring from.We are sorry that Ms [redacted] didn't understand our positionWe hope that with time she will do so by learning our side of this issueWe are sad to see her go elsewhere
