Capstone Pediatrics & Family Care

On 12/21 I requested for my 5 year olds daughter records to be released to a different doctors office so that she could get her 5 year old checkup and shots. I was told that everything was taken care of. A week later the doctor's office had not received the records and I was told that the fax machine was down. A week after the doctors office requesting the records had called capstone and was told they would email as well as fax the records. It is now January 23rd and capstone has yet to release my daughters records in order for her to get her proper shots and checkup.
