Indeed, the matter with [redacted] has been resolvedI had little choice but to come to a settlement because I am starting medical school this August at the [redacted] and I could not risk any negative credit listings hindering my chance at qualifying for student loansHowever, I don't see the matter as resolved in regard to Carent LabsIn the stipulation, the plaintiff that I agreed not to bring counterclaims against is [redacted] , the collections agencyI still may decide to make a crossclaim against Carent labs for the mistakes that were made costing me much time, effort, money, and anxietyThe fact of the matter remains that I wound up having to pay over dollars due to clerical errors made by Carent Labs after I had a routine blood draw done by my PCP in BoulderMy responsibility was to provide the correct contact information and insurance information to Carent LabsI did this, as you could see in the supporting documents I sent previouslyIt was Carent's responsibility to record them properly, which they did not doSloppy work by employees doing the billing led to these problems, and they did not bother to recheck their own work when the claim was deniedI did everything I was supposed to doI again request that Carent Laboratories acknowledge their error and reimburse me the amount I had to settle for with [redacted]

Complaint: [redacted] I am rejecting this response because:With regard to the notion of shared responsibility for contact information being up to date, the issue for me is that I had all of the correct information entered, so my part of the responsibility was coveredIt was Carent Laboratories who did not fulfill their share of the responsibility by entering the wrong address information and the wrong insurance information leading to the proposition of a dollar settlement with a collections agency for routine labsI've not yet signed the stipulation for the settlementThe only way I could have known that Carent had the wrong info was by getting some message informing me that they were not receiving payment, but I was not receiving any messages because the address was wrongI was never given some pamphlet informing me that Carent Labs specifically would be handling my labs, so I had no idea that I would need to communicate with them to ensure the insurance info and address I entered at my primary care provider had been entered correctly by Carent Labs Also, it is not true to say that Carent has done all they can to try and remedy the situation and inform me of what happenedI have tried to obtain notices of history of bills being sent out to the apartment I lived in at Boulder and the staff at [redacted] (a company that takes care of billing for Carent Labs) has been very difficult to get information fromI would hope that Carent Labs would agree to cover the full expense to take their share of the responsibility for the errors madeRegards, [redacted]

Complaint: [redacted] I am rejecting this response because: While it's true that I'm in the process of coming to a settlement with [redacted] , major blunders were made by Carent Labs in the documentation of my health recordsAfter requesting documents from both my primary care physician in Boulder and Carent labs, it was obvious that the wrong insurance information was added due to a typo, and the wrong address was enteredThese blunders made by Carent labs are going to wind up costing me dollars for some routine labs that would have been nearly fully covered by the excellent insurance I had at the timeIn my communication with [redacted] , which seems to work in partnership with Carent Labs in billing, I could not get anyone to call me back and agree to send me a history of the bill notices that must have been sent to the wrong address in BoulderI do not understand why, if they weren't getting a response from me (and presumably bills they were sending to the wrong address were getting returned to them), that they could not have A) Called me (my number was on the forms) to get my actual address, B) Rechecked the forms and noticed they had the wrong address and amend it, C) Rechecked the forms and notice that they should be sending the bills to the responsible party who I had listed as my mother at the time, and then send the bill to her home.The reason I am settling is that I need all negative credit listings to be removed from credit report as soon as possible because I will be applying for medical school federal loans soon to begin medical school in JulyAlso, I now live in Anaheim, and a trip to Boulder would cost me more money and I would have to take time off work to make the tripRegards, [redacted]

The incorrect insurance carrier was originally billed for lab work in January of I updated proper insurance information using the provided website and access key and ID on my invoice I received another invoice with old insurance information I called in to update with proper insurance information Next letter I received was from a collections agency I called into Carent again to figured out how to resolve before being turned over to collections No action was taken from Carent, my account was turned over to collections I get the run around that the billing specialist are reviewing my account every time I call in to resolve this issue Eventually my proper insurance company received the claim The remarks on the EOB say: You do not have to pay this charge That is because the provider did not send the claim to us within the required time limit The problem is I have a huge deficit on my credit report due to the lack of response and care from Carent Billing

Since July Carent has sent us multiple invoices (without diagnostic codes) stating that we are not members of *** ***We are, through ***, for many years
Since then Carent has not replied to calls, emails, or mail requesting either that they contact *** to confirm our insurance (what *** and *** told us needs to happen) or send us an itemized receipt with diagnostic codes
Carent is now threatening us with collectionsWe just want to submit the claim

A payment to this company was made in error through online banking with *** ** *** *** The payment ($1157) should have been made to *** *** but I selected *** *** *** in error from my list of payees This was done on Aug The error was brought to my attention on Sep from a cancellation notice from the insurance companyI tried to contact *** with the number I had but it no longer was their number I tried to find a website and looked at the yellow pages but nothing came up I also contacted my doctor's office but they no longer used this labI then went to *** ** *** *** to see if they could find out the bank that cashed this check to get further information for a paper trail *** ** *** *** had a hard time dealing with this request and was not helpful after telling the story different times on different occasions They finally told me on Nov they could not do anything because it was not their fault

In Oct of 2014, I had several blood tests in Boulder, CO which were handled by Carent Laboratory Solutions I had excellent health insurance which would have paid for these tests had the claims been submitted correctly Because of a data entry error*(see below) by the staff at the lab, my insurance information was INCORRECTLY recorded into their system Because of this error, the claims were not paid The lab began sending the unpaid bills to me at my CO address However, they also mis-recorded my mailing address* (see below) so that I did not receive any of the bills

I still may decide to make a crossclaim against Carent labs for the mistakes that were made costing me much time, effort, money, and anxiety. My responsibility was to provide the correct contact information and insurance information to Carent Labs. I did this, as you could see in the supporting documents I sent previously. It was Carent's responsibility to record them properly, which they did not do. Sloppy work by employees doing the billing led to these problems, and they did not bother to recheck their own work when the claim was denied. I did everything I was supposed to do. I again request that Carent Laboratories acknowledge their error and reimburse me the amount I had to settle for with [redacted].

In communication with [redacted] (Carent collection agency) we have been advised that this issue has been agreed upon in principle as follows: We ultimately agreed he will pay $723.25 to settle in full. We are going to set aside the judgment and enter a stipulation. We will...

agree to submit a deletion of the account from his credit once it is paid in full. Contacted Mr. [redacted] by phone on February 9 and discussed his billing issue with Carent Laboratory Solutions.  He stated that he had contacted his insurance company ([redacted]) and evidently they didn’t recognize him in their system and therefore he had sent a form into them so that it...

can hopefully be resolved.I tried to reach Mr. [redacted] by phone this morning however since he wasn’t available I just sent him an email with the following informaiton:    When we talked last week you had mentioned that your insurance company ([redacted]) evidently didn’t recognize you in their system and that you had sent in form into them so that it can hopefully be resolved.  In contacting our billing department on Monday they said that they still don’t show you as eligible with [redacted].  So here the detailed information they provided related to your account: 07/19/16: Patient cannot be identified 07/20/16: Checked eligibility on [redacted] website; unable to find patient 08/12/16: Verified insurance with patient and refiled claim 08/16/16: Checked eligibility on [redacted] website; unable to find patient 01/04/17: faxed claim to [redacted] 01/06/17: patient advised they will submit the claim; requested account hold 01/10/17: eligibility check prior to claim submission rejected 01/11/17: Checked eligibility on [redacted] website; unable to find patient 02/13/17: Account is on hold so there will be no further collection activities at this time Please let us know if you need any further information. Sincerely, Daryl B[redacted]
