1) That Ashton Creek knowingly permitted out-of-network medical party treatment on two separate occasions
2) That they failed to notify me of the act
22Feb16: after total right knee replacement I was sent to Ashton Creek for rehab on about 24Feb thru 05MarOn check in, I asked Ashton Creek admissions personnel to verify they were an authorized, in-network provider for United Healthcare (having already done so with United Healthcare prior to surgical date) or I would have to leaveAdmissions verified that they were in-network provider
31Oct16: after total left knee replacement I was sent to Ashton Creek for rehab on or about 02Nov thru 12NovOn check in, I again verbally requested Ashton Creek admissions personnel verify they were indeed an authorized, in-network provider for United Healthcare (having again done so with United Healthcare prior to second surgical date) or I would have to leaveAdmissions verified that they were an authorized, in-network provider
