removal from the 1199SEIU Benefit and Pension Funds Provider Network.Summary of Findings:[redacted], along with other providers, received a recredentialing mailing on 6/*/16 requesting participation status with the Fund. [redacted] responded back to the Fund with a request to stay in the network. This information was documented within our provider system with a request to begin the recredentialing process using her CAQH provider application.During this recredentialing process, [redacted], NP was erroneously terminated from the Funds network effective 7/*/2017. Her participation has since been reinstated to reflect no gap in participation.
