Unfortunately we do not have any record of Mr*** requesting a disconnect on October 12th. We apologize for this inconvenience. We did receive the email correspondence referenced by Mr*** and his services have been disconnected with an effective date of October 12th
per his email request The only charges remaining on Mr***’s account is the unpaid partial service charge for October 1st through October 12th and a $charge for the unreturned HD/DVR equipment. We have attempted to contact Mr*** to make arrangements for returning the HD/DVR. We request he contacts our office at *** to make the arrangements to return the equipment. Once the equipment is returned, we will remove the $unreturned equipment charge. A credit was also applied to his account for services extending beyond October 12. Mr*** referenced being billed for services in November. His December 1st statement has a ‘CR’ next to his service charge amounts representing credits for services. He is not being charged for any November servicesTo summarize, the remaining balance on his account is $for the unreturned HD/DVR which can be removed when the equipment is returned and $for services on October 1st through October 12th
