From: Leanne C [redacted] Sent: Friday, May 19, 9:AM To: info Subject: Case ID # [redacted] I am responding to the notice of a complaint made by one of our clients to the Revdex.com The complaint was submitted on 5/17/17, ID # [redacted] This client’s complaint is totally unfounded as they were refunded their $deposit on the same day that the patient did not attend their scheduled appointment, May 17th The patient and his mother (who I believe is the one that made the complaint), the client is an adult, both called into our office on this day and were extremely belligerent and using very foul language with our support staff over the phoneWe do not continue to engage people in this office when they choose to speak to us in this manner We have an online registration and history form that we request all patients complete prior to their first visit This form is very important for our providers to have imported into the patients’ chart ahead of time (due to the nature of our practice this information enables the providers to be able to give the patient the best advice and care that they can during their initial intake) If the information that is requested is not completed, in detail, the patient’s time and money can be wasted on the provider obtaining the patient’s history during their scheduled visit instead of enabling the provider to actually give the patient the treatment that they are seeking We were trying to obtain all of the necessary information so that the patient could have a productive visit and the patient voiced that he was unwilling to complete the registration and history form in any more detail We attempted to explain, multiple times, why this was necessary and that is when he, and ultimately his mother, became verbally abusive The conversations were ended when their abuse started and I refunded the credit card we had “on file” the $appointment deposit that was made Thank you for the opportunity to respond to this complaint I will be happy to provide any further detail if needed Leanne C [redacted] Office Manager Hendricks Therapy Myers Rd, Danville, IN Southfield Dr, Ste 1205, Plainfield, IN p 317.718.8436, x | f www.HendricksTherapy.com
