Mr***, As it has been previously explained to you, Centegra Physician Care required that you be seen by a physician prior to medications being refilled. Unfortunately, there has been a failure to comply with this and over five appointments have been canceled by you, not Centegra.
As a result of your medication refill and appointment follnoncompliance, Centegra terminated you from Centegra Physician Care. A letter reflecting this termination was sent to you in September 2016.
