I filed complaint #*** on November 17, in order for a former physician to send me my medical recordsBeen trying continuously to get these records since late August Partial records arrived on December 4, After reviewing said records, the following information was NOT sent to
me: April 25, office notes (after a minute new patient office visit, where are the notes?);I received the labs (WITH THE EXCEPTION OF PAGE 1, WHICH WAS BLANK) from that date, but not the doctor's notesNext, there is a fax cover sheet on *** *** letterhead dated May 2017, with my name and birth date faxed to ***, faxed by *** with no subjectWhat was faxed to above number; where are the contents of that fax? Next, office notes for May 2, and May 16, visits are missingNext, need typed notes (of procedure) and fluoroscopic image (as provided for procedures on July 18, & August 15, 2017, etc)Next, need typed notes and copy of images (as above) from procedures done on June 13, 2017, & August 1, Lastly, I want my MRI CD from my MRI taken on May 9, After I receive these additional records, I feel that I will have COMPLETE copy of my medical records from April 25, thru August 15, that I can take to any physician that I choose. After I receive these missing records, *** will not have to deal with me, personally, (I hope) anymore
