Review: On December 19th, the last day of physical therapy, I had asked that my billing be sent directly to my insurance company. On December 2nd, 2013 I had also put that on my application for services. As the billing was not sent to the insurance company, I was then called by the insurance company's claims manager on March 12, 2014 and was told by him that they had not received the billing. He also told me that he had called Central Washington Physical Therapy and spoke with [redacted] who said she would fax it right over. Then I called her to check on the progress of the request and she told me that it would take two weeks to get the information together. Then as I contemplated the situation, I realized that she had told me and the claims manager two different things. So I stopped into the business and spoke with [redacted] face to face and she acted like my information wasn't important and had no significant bearing on my insurance claim. She gave me the impression that me and my billing was not worth her time to look at and that I was bothering her and left me with the feeling of being discriminated against with her tone.

I then asked her when the information would be prepared, and she said she didn't know, it could be on Friday. She wasn't even sure about that. As I came home I called her back and expressed my disappointment as I also let her know that that was not fair on my behalf since the billing had already been paid and the billing had already been produced. My physical therapy is over and there have been no more additional charges on my billing since the 19th of December, 2013. So I asked to speak to her supervisor and [redacted] told me the supervisor would not be in until tomorrow, Thursday March 13, 2014. I had talked with [redacted] both on the phone and in person on March 12th, 2014. The last phone contact with [redacted] was at 4:00 p.m. on March 12, 2014.Desired Settlement: I want my information faxed over immediately on March 13, 2014. I don't think I should have to wait any longer for this simple task to be done. I want an apology from [redacted] that I think is appropriate. This interaction with her left me very disappointed in her treatment of me and I did not like the attitude she gave me.

Response:

Documentation shows that patient advised front desk personnel and treating provider to bill private insurance (three separate occasions total prior to and on the first date of treatment) specifically telling the treating provider to not bill motor vehicle insurance because they would not pay. Dates of service subsequently billed to private insurance.
