This is the only form I got in the mail. They did call me a few days before receiving this in the mailI received this in the mail on 12/I contacted them on 12/at the number provided on the sheet: 855-818-and left a message with my reference number. I also emailed them at
[email protected] never responded to me or sent me anything in the mail. The next thing I got in the mail was a collections notice from the collections agency. I called the collections agency and told them I was not happy with no official response back from Champions. Review: I completed enrollment paperwork and decided that my child was not going to attend this facility. In December, they sent me a bill totaling $276.00 for services rendered. Services were not rendered. I attempted to contact Champions by phone and email on December 14th with no response or call back to resolve the issue. Today, I received a bill from a debt collections agency for this amount from Champions. I am not happy with not getting a reply from Champions. I'm upset that this company sent this to collections and didn't bother to contact me back after I responded to their one letter about the bill.Desired Settlement: My settlement is for them to not charge me services that were not rendered. It would've been nice and respectful if they contacted me back.
