I am rejecting this response because: not only was yhe company negligent in timely response (I was able to obtain the document on my own), they never provided my insurance with any of the requested subsequential documents. The full PT eval the did along with my doctors write up for nedical necessity was never supplied to our unsurance. Therefore it was denied. Also, Champs said they had a second denial and my insurance said no new documentation was ever provided. Iwould never refer this company to annyone seeking medical supplies.

There had been a long delay because we had been waiting to hear from the insurance carrier. We inquired several times. Today, April 17th we finally received a fax the insurance carrier explaining the reason for the denial. This was forwarded to [redacted] Thanks, [redacted]...
