6/22/2016. Ms. [redacted] called customer service on both 6/27/2016 and 7/5/2016 to see what the status of her refund. Chapel hill Medtheraphy sent Ms. [redacted] a check refund in the amount of 158.95 which was returned back to our facility on 8/18/2016 as undeliverable. Chapel Hill had no way of contacting the customer for an updated address, the customer also did not provide a phone number. it was deemed undeliverable because the customer had moved. Chapel Hill, strives to provide the best customer service, we have since forwarded her check to the address provided and we have updated the address for our records.
