I have reviewed the response made by the business in reference to complaint ID [redacted], and find that this resolution is satisfactory to me.I completed the Authorization for Use and Disclosure of Protected Health Information.  This completed form was emailed to Hannah [redacted] at the Chaska Commons Dental Group today, April 10, 2016.   I have attached: the email trail with Hannah [redacted], Operations Manager at Chaska Commons Dental Group; the GEHA Dental Federal Insurance Explanation of Benefits Service date 2/18/2016 for services at Chaska Commons Dental; the Authorization for use and Disclosure of Protected Health Information sent via email to Hannah [redacted], April 10, 2016 (today).I can also provide you a copy of my GEHA benefit summary that states that Exams, Cleaning (2 per year) and X-rays are covered at 100%.  There were unnecessary X-rays and services listed on the GEHA benefits summary. There are several additional facts that I neglected to convey in my initial filing.  I requested and set up an appointment for "a cleaning".  I did not ask for X-Rays. When I arrived for my appointment, the receptionist stated that they would also do X-rays.  I questioned this and stated that we needed to check with my previous dentist first as only so many X-rays were covered in a certain period of time.  The receptionist stated that everything would be fine.
