He was paid as an out-of-network provider yet his website says that he takes [redacted] - [redacted].Desired Settlement: [redacted] provide his tax ID for his private practice as requested by [redacted](the insurance processor for mental health claims for [redacted]) so that claims can be processed for his patient's parent who carried the insurance and paid him upfront out of network.
