To Whom It May Concern: We are writing to you in response to a customer complaint ID # [redacted] regarding a billing issue The [redacted] children have been patients of our dental practice since we have always billed their dental insurance and we have never had any issues When she brought the children to their December appointments, she stated that they now had new insurance because her husband was deployed The information was provided by Mrs [redacted] and the claims were submitted in a timely fashion to the new insurance company The claims were denied and returned unpaid After a phone call to the insurance company they stated that there was no coverage nor had there been any coverage for the family at anytimeUnfortunately the wrong insurance information was provided and ultimately it is the responsibility of the patient/parent to provide us with accurate informationMrs [redacted] has contacted that there was a delinquent balance at our office and we never received a response
