November 4, 2015To Whom It May Concern: I am writing in response to your letter notifying me of a complaint against our office that was submitted by one of our patients on October 8, with reference to a problem that occurred on September 20, The patient reported that she was unable to get her medical records released to the assistance office in order for her to get health insurance,Our records indicate that the patient called our office on September 22, to check on the status of a form she had dropped off to be completed for the assistance officeThe form was signed by the patient on September 15, She provided payment of a nominal form fee to us on September 21, She called again to check on the status on September 24, The form was completed and faxed back to the assistance office on September 25, Our fax outbox shows successful delivery of the fax on that dateOn October 8, 2015, the patient called the office again to check on the status of medical records, We subsequently received a request for transfer of medical records to her new doctor's office on October 18, Her chart was sent for transfer of care on October 30, 2015, Our office makes every effort possible to deal with patient requests in a timely mannerWith regard to her complaint that she was dealt with rudely at the front desk, the issue has been investigated and staff education reinforcedSincerely,Terrance C, MD
