required to practice within the confines of HIPAA (the Health Insurance Portability and Accountability Act) and Wisconsin Statute 146.82.  Both of these laws prohibit me from providing patient information to your organization without written authorization from the patient.  At this time, I have not received the requisite authorization from my patient to disclose any information to you. I do not need to share detailed dental files. My entire medical history does not need to be part of this complaint. This complaint is regarding the yellow front teeth that are not brilliant as advertised nor do they match my adjoining teeth. The attached photos clearly show the end results and various comparisons of how they should have turned out. The end results are the issue. I think it is obvious that there is a right and wrong color for my front teeth. All the details are clearly outlined and explained in my original attached complaint document.
