For starters, Kaci R [redacted] is no longer associated with our officeJennifer O [redacted] is now the Associate Director of Operations and she can be reached @ [redacted] @chiropractic-1st.com or ###-###-####This patient presented to us after we were involved @ a Health Fair at his place of employment I have attached the flier that Kevin is referring toAs explained to Kevin several times on the phone today our physicians perform the INITIAL appointment which consists of: Consultation (meeting with our physician), Orthopedic & Neurological Examinations, Initial X-rays, and Scan Vision Muscle Testing for $In this promotion (see attached) no where does it state that patient gets results the day of or at no charge at the recommended follow up visitWe do not combine the Initial Visit and the "Report of Findings" in the same visit as this would have patients here for up to hoursDue to confusion that was reported to myself; Jennifer AssocDirector of Operations and Insurance Specialist; Katherine R [redacted] I advised Katherine to contact patient via email (as he refused to provide us with any type of phone number) to make sure he was aware of the $(our DISCOUNTED time of service fee) before coming in for his appointmentWhen Kevin spoke to our Insurance Specialist he was told that she would have to get permission from either myself or the owner/director; DrMatthew M [redacted] before she could write off any type of finances when it came to his second appointmentKevin did NOT give her any reasonable time frame to do so and Kevin called our office times within hours demanding an answer from KatherineKatherine attempted to explain several times our office protocol and was told "i don't understand" she would then again re-explainThis conversation with her (the same as my conversation with him) went in repetitive circlesPatient relayed to Katherine R [redacted] that if we did not give him his results at no charge he was demanding his money ($39.00) back as we did not give him any type of resultsAs clearly shown in the Promotion card attached: Results are NOT included in the initial visitOur clinic did exactly as we have promoted and we will not be refunding this patient for services that were already renderedDuring my phone call conversation with Kevin this afternoon he admits to having ALL services performed that were advertised on our flier for the charge of $

To Whom It May Concern, After discussing Case #: *** with our Clinic Director; DrMatthew M*** whom to which you spoke with this afternoon I wanted to send over the protocol for the 2ND VISIT: After a patient leaves from his initial consultation (when x-rays are digitally taken) our physician’s blow up these digital x-rays to further evaluate for possible fractures, dislocations, and to ensure accuracy of diagnosing a patient This can take up to hours for a physician to complete; hence why we do not reveal all results with a patient at the initial consultation. During the second visit the physician reviews all examination findings, x-ray findings, and a plan for the future is discussed with the patientIn certain circumstances if a patient is unable to adhere to our recommended treatment plan, payment arrangements can be established or the patient may be released at no charge at the discretion of myself or our Clinic Director; DrMatt M***. All expected services rendered to the patient associated with the Case/Reference number listed above were met by our clinicTherefore our decision to refund patient (for the already discounted price) stands as being declinedThis patient did not return for a second visit nor did he give us the opportunity to present findings or our clinics personalized recommended treatment plan. Out of goodwill we are willing to provide patient with examination reports, x-ray report, and our recommended treatment plan(Please be advised that this does NOT include digital x-ray images)In accordance with Pennsylvania state law, costs may not exceed $for search and retrieval, $per page for pages 1-20, $per page for for pages 21-60, and $per page for pages 61-__Based off of the records that I am prepared to mail/email to patient the value of his records are $The patient is requesting a refund of $39. I have attached the flier that Kevin is referring to. Director of Operations and Insurance Specialist; Katherine R[redacted] I advised Katherine to contact patient via email (as he refused to provide us with any type of phone number) to make sure he was aware of the $68.00 (our DISCOUNTED time of service fee) before coming in for his appointment. When Kevin spoke to our Insurance Specialist he was told that she would have to get permission from either myself or the owner/director; Dr. Matthew M[redacted] before she could write off any type of finances when it came to his second appointment. Kevin did NOT give her any reasonable time frame to do so and Kevin called our office 3 times within 1.5 hours demanding an answer from Katherine. Our clinic did exactly as we have promoted and we will not be refunding this patient for services that were already rendered. Review: Chiropractic 1st participated in our School Health fair. They offered an initial assessment for $25. After talking with co-workers, I decided to give it a try. They set up a schedule for me and before I would commit to treatment, they needed to confirm coverage with my insurance carrier. Their Insurance Specialist confirmed coverage and told me there would be no co-pay.... I assumed since I had already met my deductible and out-of-pocket for the plan year. This was the deciding factor to schedule office visits with Chiropractic 1st. I received my pre-certification from my insurance company and my visits were approved. Two weeks later as I was checking out, I was told the co-pay for that visit was $40 because January was the beginning for a new year. I told them my insurance runs on a fiscal year, not calendar year, and to please confirm again with [redacted]. I received a call from the Insurance Specialist at Chiropractic 1st and was again assured that everything was fine and I did not have a copay. Several weeks later, I received my first Explanation of benefits and noted that my coverage was coded incorrectly as a Worker's Compensation case. I called Chiropractic 1st and talked again to the Insurance Specialist. She again called me back saying that she had corrected the problem with my insurance company and that I did not owe any money. About a month later, I received another explanation of benefits, and it was noted that insurance paid most of the charges and there was indeed a co-pay. I called the insurance specialist again and received a return phone call that Chiropractic 1st had made a mistake and I now owed all the back co-pays (after assuring me THREE times, saying they talked with the insurance company and there was NO copay) I feel since the mistake was theirs, I should not owe the co-pay. Bottom line... I was not going to commit to services if I had to pay a copay. The only reason I committed was because they told me "NO COPAY" The only reason I continued was because I was told.."Yes, I checked with your insurance carrier and I assure you, you do not owe anything.Desired Settlement: The desired outcome is for them to honor their word of no copay and write off the balance.
