Dr. [redacted]'s office did not provide my insurance carrier with the information required to pay the claims made on my account despite my efforts. I am writing to file a complaint against the business "Chiropractic Memphis"(Dr. [redacted]). My problem with this business stems from an outstanding bill which I do not feel is my responsibility, as I was told by the staff that the care and procedures I received there would all be covered by my insurance. I began my initial office visits in July of 2013 (7/8/13). Initially they told that my insurance would cover 100% of my procedures. This mysteriously changed after a while. When I inquired as to why, they informed me it was because of my deductible. Fair enough, I thought - but certainly they could have informed me of this on the front end! As time went on and after more office visits, I began to receive bills for the entire amount of my procedures. When I asked why I am being billed for the entire amount and why wasn't my insurance covering this as they had told me they would, they explained that they were just waiting for my provider to pay the claim, assuring me that it had been submitted. However, the bills continued to be sent to me for the full amount. It was at this point I began to get suspicious, and promptly called my Insurance Provider ([redacted]). They told me that while it was true that the claims had been submitted by the doctor's office, they still needed addition information from them before the claim could be paid. I called Chiropractic Memphis to let them know this, and they seemed puzzled and said they would look into it. At this point (September 2013)I decided to suspend my visits to Chiropractic Memphis, as I did not want to continue receiving treatments if there was a problem with my claims being paid by my insurance provider (my last office visit was 9/11/13). I called my[redacted] again and they told me that they had still not received the necessary information from the doctor's office. I had hoped that this matter could have been taken care of by Dr. [redacted]'s office, but apparently they have failed to provide my insurance carrier with any of the needed information ([redacted] informed me that needed for them to submit a "medical treatment plan" detailing specific information about the care I was to receive). They continued to do nothing more than submit (and re-submit) claims (indeed, as of today - June 13, 2014 -[redacted] informs me that Dr. [redacted]'s office has made absolutely NO attempt at making any contact with them). So far I have paid $100 of my bill. I am not paying anything further until this matter is resolved. This is why I am turning to the Revdex.com for help. I continue to receive harassing bills from the doctor's office, when it is clearly THEIR responsibility to provide the insurance company with the necessary documents. Personally, I think Dr. [redacted] is nothing less than a charlatan out to swindle people of their hard-earned money! His tactic is simple: he tells his patients that everything will be covered by their insurance, fails to provide the carrier with the needed information, and leaves the patient stuck with the bill! Any help and assistance you could provide would be most greatly appreciated. The contact number for[redacted] is: XXX-XXX-XXXX.Desired SettlementAll charges (apart from my deductible, of which I have already paid $100.00) be dismissed/removed from my account.Business Response We will dismiss all charges apart from his deductible as he requested.

I received care at Chiropractic Memphis, under the care of[redacted], DC for a injury sustained from a car accident. After the treatment, neither[redacted] or Chiropractic Memphis responded to the request of my medical record for insurance reimbursement purpose. The insurance adjuster had sent in request on 10/2/13. He had failed to comply with HIPPA and the Tennessee State Department of Health regulation by providing the insurance my medical record. He nor the officer ever responded to the telephone inquiries about this. In addition, he made false statement to me on 3/24/14 stating the medical record is released. However, on 4/18/14, when I went to his office, he then stated he has not sent in the requested medical record.Desired Settlement[redacted] or Chiropractic Memphis should release my medical records to the insurance immediately, or he and the business should resolve me from any responsibilities for the medical bills.Business Response This complain must be removed with any coordination to Dr. [redacted] and our address of ** N [redacted] Suite [redacted] Memphis, TN 38103 as this is not a patient of our office and this is in direct violation of HIPPA. Remove our office involvement and make sure the involvement is placed with Dr. [redacted]. Thank you.
