This letter is in response to your letter dated Friday, March 24, 2016,In response to [redacted] ***'s request for a copy of her medical records.Because of the patient's rights and HIPAA laws, and to expedite the patient's response request, we kindly request the patient to call our business office directly at [redacted] (not the main phone number of [redacted] to request us to mail or fax to her the authorization to release medical records formOnce we received the signed request form, we will promptly mail a copy of the patient's medical records to the address she provides, at no charge (we will waive all fees associated with copying and mailing medical records).Please note, medical records, once a signed release form is on file, will be sent by way of certified mail via the United States Postal Service, which requires a signature.I hope this letter, which is in response to the patient’s request response, addresses all concerns to everyone's satisfaction.If you have further questions, please feel free to contact me at [redacted]

I am rejecting this response because:Im not calling.  As far as im concerned they can just send them to me.  I talked again with medicare.  Was on the phone with them and again recorded the conversation.  Again they told me they have NO record of me ever seeing this doctor.  Again if this compnay continues collection actions, I will sue them. I also turned this business into other agencies for their credit collection actions.  I hope they face sanctions

This letter is in response to your letter dated Thursday, February 25, 2016.In response to [redacted]'s complaint filed against Chitra Venkatraman, M.D., P.A., we actually rendered service to this person on the date of service in question.We submitted the medical claim to her insurance carrier, and...

after receiving reimbursement from that carrier, we billed the patient for her 20% responsibility. We billed this patient several times since 2014, and on November 03, 2015, mail was returned to us by the United States Postal Service with a forwarding address for the patient, We billed to that address, and on February 10, 2016 around 11:20 a.m., [redacted] called us, stating she wanted us to write off the balance, After informing the patient that we will not write off her balance because it's her responsibility, she became disappointed and stated she was going to report us to the Revdex.com and ended the call abruptly.If this patient wants a copy of her medical records as verification that we rendered service to her in our office, she can call our office to request a release of medical records form, and once she signs it, we will gladly provide her with the proof.
