I saw Dr. *** in August of 2019 at his Southlake office as follow up to prior DVT and prescribed indefinite use of xarelto. Intent of visit was to discontinue use of xarelto.
Dr. *** ordered two blood draws and both were done as ordered.
I received a *** form LabCorp for $654.83. I contacted my insurance company, Blue Cross Blue Shield NC as was told that no prior authorization had been obtained by Dr. ***s' office. I was told that if PA had been obtained, I would only be responsible for a small portion (app $30) of total due
I contacted Choice regarding the ***. *** returned my call and requested EOB. I emailed EOB to her on 11/29/19. *** responded quickly and said "Thank you Mr. I will see what I can do to correct this."
I have followed up with *** by email on 12/23/19 and 1/23/20 but there has been no response. My wife left her a voicemail on 1/31/20 and there has been no response to that either. f
Hello, the lab sent a *** to the patient stating a prior authorization was required before services were rendered. Our office had no knowledge about the prior authorization until the patient contacted our office regarding the ***. I contacted LabCorp and provided additional information for them to submit to the insurance carrier for review. The test ordered were medical necessary and appropriate but we had no knowledge that the insurance required a prior authorization for the service. LabCorp agreed to suspend ***ing the patient and send an appeal to the denial for additional review based on medically necessary treatment. Thank you
