We signed a contract with Kandace Perryman agreeing to pay $120/hr for counseling services. After several months of her "working with our insurance company" we were presented for a bill for thousands of dollars and charged $175/hr. We have asked her repeatedly to reissue the invoices for us and she claims that the $175/hr is her agreed-upon rate with insurance. *** has no record of talking with her and no agreed upon, contracted rate with her. She also charged us $300 for two hours of evaluation sessions for each child, even though she only spent one hour with us ($150/hr).
Please note, due to HIPAA compliant regulations, there will be no attachments submitted unless *** wishes to sign a release of information form allowing Revdex.com access to their personal information.
All claims for patients are billed using a usual and customary rate that is set at each office's discretion for their geographical area.
All claims were billed to ***'s insurance company. All the explanation of benefits received from her insurance company stated that payment had been made to to the insured.
Once we received this notification, we sent the patient a statement requesting submission of payment to us.
We did not mail the patient statements for thousandS of dollars.
After we were able to confirm that the patient did not receive payment, the patient was sent a revised statement with agreed upon initial session at $150.00 and the follow up sessions of $120.00 on March 2, 2020 at 3:56 pm.
