provided was for a date of services of 11/30/13. This account has a zero balance. There next date of service available was 3/25/15. The total charges were $1236.00 with the patient balance of $185.40. All hospital correspondence for this date of service has been returned to our billing department as the patient provided an incorrect address and left prior to registration.
