determined by the insurance company was $4416.00. The insurance company paid $3974.40 which is 90% of the allowable. The remaining $441.60 is the 10% copay. Therefore the Consumer owes a 10% copay of S441.60. We have mailed a letter of explanation to the Consumer. We are more than happy to work with the consumer and set up a payment plan if needed.
