My wife and I have been going to this dentist for about 5-years I have never had any problem before this incident The incident started when I received a bill from themOn the bill it has Periodic oral evaluation (general cleaning) for $and another charge for Periodontal maintenance for $ This Periodontal maintenance is not 100% cover by my [redacted] insurance so my out of pocket for this bill is $ I called the office to find out why this was the case and the first person picked up could not answer and she refused to give me her manager's name or when the manager be arriving to the office She said "it's violating privacy law to tell clients when the manager is going to be in the office" I insisted that she would give me a time frame and she said someone will get a call back to me within the hour I got a call minutes later from a hygienist who knows me and she said to me that the Periodontal maintenance was necessary every three months and for four times after I had my deep cleaning back in July I told her that I understand that's the recommendation to patients but I wasn't aware that was happening for the past four years She did not have anything else to say besides "we've been doing that for you since 2013." That was a direct quote I asked her what's the different between a regular cleaning and a Periodontal maintenance and she said they're kind of feel the same So, I said there's no proof that it was done on me? I didn't feel any different nor anyone told me differently when I came in every six months She then told me someone else will give me a call back I got a call from a third person name Deanne She then explains to me why I needed the Periodontal maintenance and kept harping on how it's a policy to do it She has all the answer in the world to defend the office and kept saying "we have notes, our hygienist put it in the notes" I was trying to tell her that I wasn't aware that was happening but she was busy giving me all the details on how her office has dental procedure and what needs to be done after a deep cleaning All I wanted was to know WHY WASN'T I AWARE OF THIS at any point since She kept saying that it was best for our clients and yet she could not tell me why it wasn't disclosed to me Another thing she told me is that the bill didn't get to their accounting department and it did not have the appropriate discount on thereShe didn't know why I got that version I requested that she print out a ledger of all my charges and she said "Belinda will call you back and print that out for you" Belinda called me short after and explained that the bill I got was correct $of the $is deductible and $was for copay Then she explained that's the case because I had the Periodontal maintenance and my insurance only covers 80% She said all the other times the insurance discounted the $ It must've been my insurance that change I explained the whole situation to her but she said the hygienist is responsible for deciding what service to recommend me She then told me that this was coded on my bill to Kaiser insurance for the past four years, which equals to about SEVEN TIMES of Periodontal maintenance She then told me she could take off $and that's all she can do At this point I was frustrated so I told my wife to go pick it up the next day She spoke to Belinda and explains why I was upset but no one there could look her in the eye or to say anything to her Belinda knows my wife personally and my wife usually is the one who call to schedule a teeth cleaning appointment EVERY SIX MONTHS She usually will ask if anything will cost out of pocket And I remember hearing her once at the dentist office that we do not want to do that deep cleaning maintenance (Periodontal maintenance) because it will cost us out of pocket All in all, we will be switching dentist but I wanted to write a letter to you to explain why this office is unethical and might be violating insurance fraud They did not acknowledge it was a misunderstanding or admit that maybe one of them forgot to tell me in the beginning that a Periodontal maintenance was performed on me in the SEVEN TIMES I was there for the past years Even though they said they have notes but I do not trust that since I wasn't disclosed or feel the difference between a regular exam/cleaning versus a Periodontal maintenance If I didn't get that bill or Kaiser insurance didn't decide to "discount me" that $then Kaiser would've pay for Periodontal maintenance whether I got it done or not I sincerely hope you take this matter seriously

We don't have a camera in Front Desk so we can't get her tape when she arrived and checked-out. We don't know if Ms. [redacted] stayed in the room for the entire time once she checked in. Her claim for a leaked rug in the room is totally false. You can also check our review score online and we haven't received a single complaint about any issues that Ms. [redacted] stated.
We decide to give her refund only if she can provide us with her Doctor's report stating we are solely responsible for her illness.
