[redacted] to [email protected] via email. First allow me to say on behalf of Clear Lake Regional Medical Center we are sorry we did not meet the expectations of the complainant. We take all matters and feedback very seriously and the report of a concern helps us identify areas, if any, that need improvement. We appreciate when concerns are provided to us. It is alleged that CLRMC did not correctly update a patient’s address in the hospital database ultimately resulting in the patient’s account sent to collections, and according to the patient, negatively changing her credit score. A credit deletion request was submitted as a courtesy to the patient and a notice sent to the FCA to submit for credit deletion to the bureau. The Patient Account Services Department reports this should correct any issue related to the hospital account on the credit report. Our team strives toward providing high quality, safe patient care and we also want our patients to be satisfied. To that end, the complainant will also be provided with a response to her concern. Sincerely, Cynthia [redacted] Cynthia [redacted], RN, BSN, BA, JD Manager of Risk Management

From: Cynthia.G[redacted]@hcahealthcare.com [mailto:Cynthia.G[redacted]@hcahealthcare.com] Sent: Monday, January 16, 2017 7:20 PMTo: drteam <[email protected]>Subject: Response to Case # [redacted]
To Revdex.com of Metropolitan Houston,
As you instructed in your most...
recent email response to my inquiry, I am responding to complaint ID [redacted] to [email protected] via email.
In complaint ID [redacted] it is alleged the complainant is receiving invoices for services rendered that were already paid in full. It is also alleged that the complainant previously sent the same concern to the Revdex.com and Clear Lake Regional Medical Center had previously responded. We do not have on record the complaint was received and addressed previously. That doesn’t mean it wasn’t, but this writer does not have a previous complaint or response on file. We do apologize if that is the case.
Please allow me to say on behalf of Clear Lake Regional Medical Center we are sorry we did not meet the expectations of the complainant. We take all matters and feedback very seriously and the report of a concern helps us identify areas, if any, that need improvement. We appreciate when concerns are provided to us. It is our policy to review all concerns brought to our attention through our internal review process. These reviews are quite thorough and while this review process is confidential we want to respond to the concern.
The hospital has on record the account was paid in full at the time services were rendered. The account has been adjusted and is a (0) zero balance. Thank you for alerting us of the receipt of invoices. There should be no further collection notices received by this complainant.
As stated earlier we do regret not meeting the complainant’s expectations. Our team strives toward providing high quality, safe patient care and we also want our patients to be very satisfied.
Yours Truly,
Cynthia G[redacted], RN, BSN, BA, JD, CPHRM
