Hello, We have discussed this issue at length with our patient She was informed before treatment was started that we do not accept or provide information to insurance companies, and that there was no guarantee any insurance company can reimburse her for services rendered hereWe do not have any relationship with the insurance companies and are a fee-for-service only practice This means that we do not provide signatures on claim forms or tax identification numbers As we do not accept assignment of benefits from any third party or insurance company, insurance companies are not required to have this information from us Unfortunately, her insurance company is erroneously insisting that they do need this information that we simply cannot provide as a non-contracted provider that accepts no payment from insurance companies or has any interraction with them

was useless as my insurance company would not accept it. As I understand from the insurance company, the tax id number is requested by insurance companies as a way to determine if the provider is a part of their plan or not in order to determine the proper payment to the insured. It does not have any effect on the dentist or his practice. The payment would not go to him without the insured's permission. (This is the reason I was given for them not releasing the tax id number.) We appreciate the opportunity to respond. The claimant was seen for medical treatment and as such, given the confidential and sensitive nature, our response is limited in detail out of respect to the the privacy of the matter. The claimant had a dental implant procedure performed in 2016 with...

ClearChoice Dental Implant Center, Edina. After the claimant's procedure was performed, she returned for multiple follow up visits as scheduled, and additional visits at her request. At each visit, the claimant had opportunity to express any concerns or needs, and her concerns were addressed to the extent possible. We believe the practice went above and beyond to satisfy any of claimant's concerns through same day scheduling and treatment. In an effort to fully satisfy the claimant, this was done without additional charge. At all times the appropriate risks were discussed and proper consent forms were reviewed and signed. In the time since her procedure, the practice has seen the claimant for numerous additional office visits to address each and every concern she may have had at the time. While we would be open to your suggestions, we believe we continue to do everything possible to resolve the claimant's issues within the same day.  We continue to contact the claimant and had scheduled her appointment and same day repair on September 12th.  Regards, Jennifer [redacted] Operations Manager
