L*** Inquired about CTIs Phlebotomy program January 29, 2014; she received
the program information and had her questions answered by Primavera Fuller,
Campus Director. MsL*** then scheduled an enrollment
appointment for February 1, for Saturday
Phlebotomy class starting
March 1, During the enrollment process MsL*** paid her
tuition of $in full and had the opportunity again to ask questions
regarding the program and at no time was MsL*** told that the classroom
schedules or externship schedules are flexible; as this is a direct
contradiction of CTIs Orientation packet (Please see images below).
CTIs
policy is to send a series of emails to the students throughout their training;
these emails provide the students with information regarding their externship
options and specific site requirements. Ms
L*** was referred to me Patricia Espinoza, Corporate Compliance Officer/
Externship Director by MrJon Davis, Executive Director as this is the proper
protocol MrDavis does not handle externship issues. The
following is a timeline of the numerous attempts by CTIs Externship Department
to place MsL*** in an externship
June 3, 2014: Ms
L*** was offered an externship at
Quartz Hills for September 6, 2014; the first available Saturday. MsL*** refused the site because she did
not want to wait, she wanted to start immediately, A*** ***, Externship Coordinator
explained to her that this is our only Saturday externship site and students
have already been scheduled in advance from prior classesShe was then offered an evening externship site for June
16, at Community Memorial Hospital, pending the remanding externship
site requirements (as explain in the series of emails sent). MsL*** refused this site as well because
she informed A*** ***, Externship Coordinator she had something to do
that day
June 16, 2014: Ms
L*** was offered an externship in Lancaster
October 11, by Sharon Calimano, Externship Coordinator the 16th
of June via email. MsL*** did
not acknowledge the email.August 5, 2014: Ms
L*** was then offered another opportunity at Quartz Hills with a tentative date of January 3, 2015. Unfortunately Quartz Hills informed us on
September 18, of the need to push back externship schedules a few
weeks. MsL*** was notified and
agreed to the changed; the same day via email and rescheduled for January 31,
2015; and reminded again per the orientation packet; CTI has no control over
the sties schedules, Students must be Flexible.January 19,
2015: Ms
L*** contacted MsNuusolia informing her that she would need to leave
early the first day of her externship and wanted permission to do so, Ms
Nuusolia informed MsL*** we do not make the schedules and therefore
cannot give you permission to make changes; only the site preceptor
can. MsL*** was advised to speak with her on the first
day. MsL*** stated she was not going to drive out there if she
would not be able to leave early. She would be calling the site prior to
starting. MsNuusolia contacted Isabel Andrade, Quartz Hill preceptor to
let her know a student would be calling her regarding her Saturday externship
schedule. MsAndrade informed MsNuusolia that if the student could not
commit to the schedule it is best for her to be placed at a different
facility and requested other students for that week. January 20, 2015: Ms
L*** contacted me January 26, 2015; where we discussed the reason she
was terminated from the program. I referred her to the Orientation packet
where it states students must be flexible; CTI does not set the externship
schedules. Also, if the student does not show up for their assigned schedule
they are self-terminating from the externship. I further explained to her
that both externship coordinators have offered her numerous opportunities for
an externship, but due to her personal schedule she could not commit; for this
reason a wavier was sent. Per
our phone conversation MsL*** termination would be rescinded and I would
be assisting her directly with externship placement. I also, made
it perfectly clear that currently we do not have Saturday externships and an
alternative option; since she mentioned she has a lot of connections in the
medical field, and her boyfriend worked at the VA hospital is to find her own
site with our assistance in approving the site
April 2, 2015: MsL*** was emailed a detailed list of
available externship sites to choose from.

April 13, 2015: MsL*** was offered an externship for June 8, 2014; which she was originally scheduled
on June 16,
April 14, 2015: MsL*** chose an externship for May 4,
at Fatima Medical Clinic A South Alvarado, Los Angeles, CA 90057. Ms
L*** started her externship At Fatima Medical Clinic, where she did not
return from lunch instead contacted CTI said she was not going to return to the
location because she didn't like it; I informed her that she self-terminated
from the program and would be receiving a Termination letter. I also
informed her at this point CTI has met their obligation to her by providing her
with an externship and she would need to locate her own site in order
complete her externship.
In
summary: MsL*** complaint about Fatima Medical ClinicWe have an
excellent working relationship with this site and have never received
complaints by students. Externship sites must be approved by the
state and our insurance provider; which can take up to days. The State
of CaDepartment of Public Health Laboratory Field Services approved Fatima
Medical Clinic in October as an externship site for CTI. Fatima
Medical Clinic requires a negative TB test and scrubs; Spanish is not
required; we currently have several externship sites under contract that
require a student to speak Farsi, Spanish, or Hindi.
CTI
has met their contractual agreement to MsL*** by offering her several
externships opportunities and providing her with one she specifically
choose.

MsS*** completed her classroom training on the 12th of March; she was scheduled for an externship to start April 27, Unfortunately the externship site called S*** ***, Externship Coordinator and informed her that they would have to canceled all externships for the next three weeks;
students are made aware that CTI does not make the externship schedules, students must be flexible (Please see attached orientation)MsS*** was then offered another externship at a site miles from her homeMsS*** was also informed that it would take her business days to complete the state requirement of hour and clinical draws at this siteMsS*** refused the externship being offered due to the distance; she did not want to drive that far
MsS*** was informed of another externship site in Bakersfield which had specific requirements; these requirements were emailed to her on Tuesday, March 17, 12:PMOnce she meets these requirements she will be scheduled to start an externshipTo date MsS*** has not turned any paperwork for this site

policy is to send a series of emails to the students throughout their training;
these emails provide the students with information regarding their externship
options and specific site requirements.  Ms.

protocol.   Mr. Davis does not handle externship issues. 
The
following is a timeline of the numerous attempts by CTIs Externship Department
to place Ms. L[redacted] in an externship.

16, 2014 at Community Memorial Hospital, pending the remanding externship
site requirements (as explain in the series of emails sent).  Ms. L[redacted] refused this site as well because
she informed A[redacted], Externship Coordinator she had something to do
that day.
June 16, 2014: Ms.
L[redacted] was offered an externship in Lancaster
October 11, 2014 by Sharon Calimano, Externship Coordinator the 16th
of June 2014 via email.  Ms. L[redacted] did
not acknowledge the email.
August 5, 2014: Ms.
L[redacted] was then offered another opportunity at Quartz Hills with a tentative date of January 3, 2015.  Unfortunately Quartz Hills informed us on
September 18, 2014 of the need to push back externship schedules a few
weeks.  Ms. L[redacted] was notified and
agreed to the changed; the same day via email and rescheduled for January 31,
2015; and reminded again per the orientation packet; CTI has no control over
the sties schedules, Students must be Flexible.
January 19,
2015: Ms.
L[redacted] contacted Ms. Nuusolia informing her that she would need to leave
early the first day of her externship and wanted permission to do so, Ms.
Nuusolia informed Ms. L[redacted] we do not make the schedules and therefore
cannot give you permission to make changes; only the site preceptor
can.  Ms. L[redacted] was advised to speak with her on the first
day.  Ms. L[redacted] stated she was not going to drive out there if she
would not be able to leave early.  She would be calling the site prior to
starting.  Ms. Nuusolia contacted Isabel Andrade, Quartz Hill preceptor to
let her know a student would be calling her regarding her Saturday externship
schedule.  Ms. Andrade informed Ms. Nuusolia that if the student could not
commit to the schedule it is best for her to be placed at a different
facility and requested other students for that week. 
January 20, 2015: Ms.
L[redacted] contacted me January 26, 2015; where we discussed the reason she
was terminated from the program.  I referred her to the Orientation packet
where it states students must be flexible; CTI does not set the externship
schedules.  Also, if the student does not show up for their assigned schedule
they are self-terminating from the externship.  I further explained to her
that both externship coordinators have offered her numerous opportunities for
an externship, but due to her personal schedule she could not commit; for this

be assisting her directly with externship placement.   I also, made
it perfectly clear that currently we do not have Saturday externships and an
alternative option; since she mentioned she has a lot of connections in the
medical field, and her boyfriend worked at the VA hospital is to find her own
site with our assistance in approving the site.

available externship sites to choose from.

L[redacted] started her externship At Fatima Medical Clinic, where she did not
return from lunch instead contacted CTI said she was not going to return to the
location because she didn't like it; I informed her that she self-terminated
from the program and would be receiving a Termination letter.  I also
informed her at this point CTI has met their obligation to her by providing her
with an externship and she would need to locate her own site in order
complete her externship. 
In
summary: Ms. L[redacted] complaint about Fatima Medical Clinic. We have an
excellent working relationship with this site and have never received
complaints by students. Externship sites must be approved by the
state and our insurance provider; which can take up to 60 days.  The State
of Ca. Department of Public Health Laboratory Field Services approved Fatima
Medical Clinic in October 2013 as an externship site for CTI.  Fatima
Medical Clinic requires a negative TB test and scrubs; Spanish is not
required; we currently have several externship sites under contract that
require a student to speak Farsi, Spanish, or Hindi.

Review: I completed my course March of 2015. In the middle of March I received an email from S[redacted] confirming my externship for 4/7/15. Days before the date of the externship I receive a phone call saying it was canceled and I couldn't get an externship again until the end of June. I disagreed and thought this was unfair, so after some online research, I called the corporate phone number & asked to speak to J[redacted] the owner. That same day I received a phone call from Kevin T[redacted] the Regional Director concerning my request. After explaining the situation he stated there wouldn't be anything he could do, but that he would look into it and that most likely [redacted] had not received with enough time the notice of cancelation from the externship cite. While I waited for a call from Kevin, I found an internet source, & looked back at my Emails to make sure the dates I had were correct, It was at that moment that I found an email from [redacted] dating A week prior the telephoned notification. Shortly after I received the phone call from T[redacted], stating that [redacted] received notice two days prior to contacting me, that was clearly a lie. After seeing that both T[redacted] and [redacted] were in a position opposite of helpful, I asked to speak to J[redacted], the owner so he could resolve my problem, T[redacted]s words were "No, you do not need to speak to him, you speak to me," to which my response was "well you are not resolving my problem so please transfer me to a supervisor or someone who could resolve my issue." his response was again "No, there is no one else for you to speak to, you speak to me." Later, I received a call from [redacted] explaining that she could set me up with externship in Shafter, where I wouldn't complete the draws necessary to become certified & would later on have to complete another 40 hours of externship to finish. I denied it since it was out of town, and they said they'd put me at the end of the list. Today 7/16/15 I called again to see when they were planning(cont'd)Desired Settlement: to schedule me, S[redacted] said availability wasnt until late Sept so I asked about the requirements for the hospital and the price, she said she'd email me, after I told her I didnt have access, she said she didnt know off the top of her head and could not help me.(There are more relevant details but due to the restriction of characters I am unable to finish the complaint)I would like an externship right away or my money back I paid $1,894 cash

As this time CTI has met their obligation to Ms. S[redacted] by offering several opportunities to complete her externship. CTI will continue to communicate with Ms. S[redacted] and offer her the opportunity to complete her externship.

Review: I registered to take a Phlebotomy class with this school in Woodland Hills. The instructor was great, very knowledgeable and passionate about the course. The school faculty is another story. After completing my lecture courses it was time for the externship. When I initally registered for the program I was told that the schedule would be flexible so I registered for an externship in Lancaster. The program coordinator e-mailed me numerous times making changes to my scheudle and at one point I let Sharon " externship coordinator" that I was not able to continue rescheduling at the mercy of the school as I was requesting time off to complete this externship. Sharon attempted to kick me out of the program when I let her know that I was not able to continue recheduling due to the schools scheduling issues. I then contacted the school director " Jon Davis" who never had the decency to communicate with me directly regarding the situation, instead he bounced me to P[redacted] (who was very professional and easy to work with). P[redacted] re-sent me a list of externships and gave me the times and dates so I ended up choosing that fit my scheule and closest to my home. My first day of externsip was May 4th was day 1 of my externship training at the facility in Los Angeles called Fatima Medical Clinic. The staff and patients all speak Spanish and I do not know Spanish. I feel as though this should have been communicated to me by their staff as I would have never chosen to go to a facility where I was unable to learn anything or communicate due to the language barrier. The training facility was unsafe for me and the patients. The facility was not following procedure, the order of draw was not followed and I was referenced to several times as"gringo" throughout the course of my day. The facility was filthy, they left specimens randomly lying around at the counter, they had an old console TV holding up a bed where they were doing ultra sounds right next to the phlebotomy chair.Desired Settlement: I want all of my money back so I can go get my education and certification elsewhere. I gave the school many opportunities and they have proven to me that they are extremely unprofssional and do not have good business ethics or practices. I wish to cut my ties and re-register with another school to retake this training.

Review: My teenage (17) daughter enrolled in the phlebotomy class and was told that upon completion of the class and an intern/externship, she would be able to take and pass the state certification test to become a phlebotomist.We were warned ahead of time that the school didn't always have a 'plan' for the intern/externship so prior to signing up and paying for the course, we asked and made sure that they had that part of the class covered and were assured they were prepared.We are now approaching a month since the classroom part of the training was completed and there is no 'plan' for this intern/externship. Any attempt by my daughter to talk to the local hospital about getting this training is met with a phone call admonishing her that she is breaking protocol and she should stop.When I call, as her parent, [redacted], the voice on the answering machine, also calls my 17 year old daughter and acuses her of 'breaking protocol' and tells her he cannot talk to me since she is the student. He took money from me but can't talk to me.Anyone considering this needs to get the entire agreement in writing complete with dates of completion or go to a better backed school that doesn't double as a traffic school.Desired Settlement: I would like her to complete the training and certification we paid for or explain why if the school cannot provide the extern/internship, I should not get a refund.
