*** *** came into Club Dental and signed up to pay for (# 2,6,7,surgical ext) (#8,9,11,simple ext) and an upper immediate denture; as sent above in her contractAll of ***'s payments were declined to do insufficient fundsWe tried eight times on her card which was on her contract
to get paymentsPt came in to the office on Jan 12, and only paid in cashAt this point pt was behind and was told she has to become current on acct to have any other work donePt never came back or paid for her servicesPt was sent to collections March 20,We gave *** five month to become current on acct which she failed to do. Tell us why here...On 03/28/the patient *** *** came into our South Jordan office to have a root canal done on one tooth, #She requested to be sedated for the procedure due to anxietyOur doctor called a prescription into the Smiths pharmacy, in West Jordan on Redwood Road, to be used
for sedationShe took the medication as directed prior to her appointment then came into the office with the remaining medication to be given to her by the providerThe procedure and sedation both went great while she was in the office she showed no signs of being sick or anything to be concerned aboutShe was monitored by the provider and his assistant throughout the sedation and afterwords till her ride was there to transport her homeNotes from the provider and his assistant:03/28/2017:Pt came in today for two RCT/BU/CRN. Pt has requested conscious sedation and followed through with picking up medications necessary for sedation. Applied topical. Pre op impressions acquired. Used carps septo & carps lido. Prepped # RCT: canals; DB-DL-ML-MB-18.#took a long time to find all the canals, so we will have to wait to prep #at next appointment. Prepped BUUsed primer/bond & fujiCured & adjusted as needed. Final impressions acquired and sent to method lab. fabricated temp crownsadjusted as neededUsed nextemp cement cleaded as needed. NV: seat Crown #prep RCT for #3. KE* Pt has an autistic yr old daughter. For ***'s appt,she arrived only taking 5mg of Diazepam almost mins before appt time; she did not take any Diazepam the night before. 8:30 10mg Diazepam, 0.125mg Triazolam sublingual8:50 5mg Diazepam, 0.125mg Triazolam sublingual9:20 10mg Diazepam, 0.125mg Triazolam sublingual.We began work on pt at 9:45. Pt was alert and in no distress. Pt periodically closed eyes, but did not lose consciousness at all; although she was quite tired Appt ended at 11:40. By appt end, pt was very alert and eyes were alert and open the entire time. Pt got out of chair without help, used the restroom and wanted to walk downstairs; we had her use a wheelchair as a precaution even though she wanted to walk DB
