Response:
Our computer system erroneously issued two invoices for the renewal of the Professional Liability policy of [redacted]. Both invoices were paid by [redacted]'s employer. Our computer system also issued correspondence to the policy holder that the policy would be cancelled for non-payment after these payments had been received.
The overpayment has been refunded to the employer with our deepest apology for the inconvenience they and the policyholder have experienced.
We have fixed the computer 'glitch' which initially caused this problem.
