I am writing on behalf of myself to appeal the decision to deny the request for services on September 29-30, 2019 because the services were found to not be medically necessary. If you look at my extensive medical history and necessity of the peripheral nerve stimulator, you will come to understand it’s initial placement for the management of chronic pain of peripheral nerve origin. Due to the placement of the peripheral nerve stimulator, the following few days lead to exacerbation of pain, nausea, emesis, fatigue, and severe drowsiness, which had significantly decreased my quality of life. Therefore, I am appealing the decision to deny the medical necessity of the removal of the peripheral nerve stimulator as the peripheral nerve stimulator did not successfully manage the chronic pain I have been experiencing, rather it further debilitated my well being.
