Review: I recently purchased a Groupon for:

The Groupon stated that for $35.00 I could go to Cohens for an Eye Exam and get $200 Toward Frames and Lenses ($260 Value)

Clients get their vision checked and select the perfect pair of prescription eyeglasses or sunglasses fit in nondesigner ($50–$250) or premium frames made by Prada, Coach, and Michael Kors ($250–$550).

With my Groupon I visited the Cohens at: [redacted]

I got my eyes examined and bought a pair of Burberry eyeglasses. Because the office was closing soon the Doctor asked me to come back to have my eyes dialated. I told her I would. Meanwhile, a saleswoman told me that if I bought a 2nd pair of eyeglasses it would be half the price. I told her that I did not want another pair. She told me that my health plan would probably pay for it and asked for my insurance card to see how much Oxford would pay for the 2nd pair. She came back with my medical card and told me that my insurance would pay $100.00. After paying $350.00 for the Burberry I told the saleswoman that I was not interested in purchasing another pair.

I visited my Oxford webpage and found out that [redacted] of Cohen’s Fashion Optical in Jackson Heights sent a claim totaling $810.00. I called Oxford right away and they explained that they did not pay Cohens as the claim type was out-of-network. Whether or not Oxford paid Cohens is not the issue, the issue is why did Cohens put a claim in when I used my Groupon? I never went back to get my eyes dilated since I now do not trust the company.

Thanks for listening!Desired Settlement: I do not know if this is a crime. But I do know that it is not right. My desired outcome would be to get a full refund because they still owe me the 2nd half of my eye exam ([redacted]). I am never going back.

On October *, 2013 [redacted] came into our facility with a Groupon. The Groupon voucher includes a routine eye exam (worth $60)i and $200 towards a complete pair of eye glasses.

Before any patient receives an eye exam they must fill out a "case history form". This form is a series of health related questions that coincides with visual acuity and eye health. Since many illness or medications car, effect a patient's vision and cause eye disease. [redacted] responses on the "case history form" warranted a more extensive examination. We have the latest equipment and technology on the market to do an extensive medical examination. The Doctor is able to get a good look at all aspects of the eye, including the back of the eye.

[redacted] received a more extensive exam without dilation. The dilation is when drops are placed in your eyes to dilate, or widen, the pupils. The Doctor uses a special magnifying lens to examine the retinal to look for signs of damage and other eye problems, such as diabetic retinopathy or age-related macular degeneration. A dilated eye exam also allows the doctor to check for damage to the optic nerve. Dilation usually occurs in 20 to 45 minutes after the drops have been placed. Since [redacted] was the last patient of the day the customer was asked to return the next day for dilation.

The customer's health insurance was billed for services rendered only. The dilation was never billed since it was not preformed. [redacted] received a routine eye exam

($60.00 value) and eye glasses were furbished with $200.00 credited towards the eye glasses as advertised on the Groupon') voucher.

Please be advised that [redacted]'s medical insurance was billed only for the extensive medical eye exam and procedures rendered. Please see attached signed "Patient

Authorization Form" for medical services rendered.
