examined [redacted] I gave her a written quote of [redacted]$ for a surgical repair of Prolapsed third eyelid gland.I went over the itemized written quote with [redacted] during the appointment. I verbally told her in the appointment that the exam fee today WAS NOT INCLUDED and I told her she would be charged for that today. THERE WAS NO MENTION BY MYSELF or ANYONE ELSE OF ANY REIMBURSEMENT WHAT SO EVER OF THE EXAM FEE. She paid the exam fee on her way out and booked the surgery for April 17/17. To verify this I had a written detailed quote with all procedures itemized, so there would be no misunderstanding. She scheduled the surgery. When she admitted [redacted] for surgery April 17/17 She signed the consent from with the itemized fee. Nowhere in that quote was there a reimbursement or minus of **$ plus tax. By signing the quote she agrees to the fees above. I can provide this document as well as the email, however I need the clients permission to release any written information from her file. Please have [redacted] send an email to the clinic giving us permission to release the email and signed consent form and I will send it to you ASAP. Sincerely, [redacted]
