On August 18, 2015, we received a signed request to cancel the policy effective 8/17/After policy cancellation there is a remaining balance on the account of $Because your policy effective date begins the 13th of each month but your chosen payment withdrawal date is the 17th of each month, a portion of the $is for the coverage provided from 8/13/to 8/17/The remaining portion is for policy changes done earlier in the term that were done in between billing cycles
