Evaluation by our front desk
staff on 11/16/to determine the nature of his complaints and if he was an
appropriate patient for our practiceAll demographic and insurance information
was obtainedIn addition, as per office policy, credit card information was
obtainedThe patient was informed that this creditcard would be used to payfor a missed or canceled
appointment in the event he did not give hour business notificationThe
patient clearly understood this or he would not have given the credit card
information and we would not have recorded it and scheduled him.Mr*** scheduled a hour Psychiatric
Evaluation on 12/11/17, but did not show for this evaluation as he apparently believed his intakeappointment was 12/14/He had received a confirmation/
reminder robo-phone call on 12/08/17, but apparently did not listen to the
message or a message was allegedly not recordedOur computer indicates the call went throughHe was informedon 12/11/by our frontdesk staff that he had missed his appointment, and
that a no show $fee would be chargedHe rescheduled his appointment for
01/02/but then canceled that appointment and has never been seen.We then sent an invoicefor his no show appointmentWe did not charge his credit card on fileHe sent a check along with
the complaint that he recorded with the Revdex.com and
DORAWe have never cashed the check.We are certainly not "running a scam" as Mr***
allegesHe was informed that our office
policy is to charge for no shows or late cancelations as a significant block of
time was set aside for himI certainly understand his concern that we or he recorded
the date wrongI am glad not to cash his check, and not to charge him for the no show andwould have figured out some reasonable solution to this problem ifhe had
talked more with us and still wanted to be a patient and be seenAt this
point, it would not be appropriate for him to be a patient in this clinic
