In response to your inquiry related to the above report the medical record has been reviewed and found to be appropriate for the presenting symptoms at the time of the patient’s visit. She was seen on 1/18/2016, filed the complaint on 2/1/with you, contacted our patient financial
representative on 2/2/for assistance in completing a charity care application. This was reviewed and is awaiting final approval as of 2/9/(the same date the complaint was mailed to us). This will leave the patient with no balance due which was her request. I would question some of the facts of the patient’s recollection and other insinuations but the outcome is as she had desired. We consider this matter resolved
