We have left messages for Dr [redacted] and are awaiting his call for further information We need the information from Dr [redacted] to validate the request for refund, as all of our services are non refundable and we would be making an exception for Mr [redacted] Specifically what contraindications utilizing our services has with his care from his primary care physician Regards, [redacted]

We have spoken with our patient and explained the day he signed up for service that he is signing up for a time based treatment It is up to him to take full advantage of the options we include during his treatment We discussed early on with the patient that we would put his treatment on hold as a courtesy The patient followed up by stating he could not use our treatment, we advised our patient that we would need something in writing from his doctor explaining the medical contraindications to our treatment that he elected to pursue At this time we have not received that information We have explained to our patient that we can not move forward without it

Saying the service is non refundable is a total lie! In my consultation *** assured me and my wife we could get a refund with no problemHe said I would have to get a note from my Drwhich I have doneNow they want more infoMy Drhas not received any callsMy specialist Dr*** said he would send them a letter alsoThis company is doing all that they can to get out of this

We have reached out to Mr*** in an effort to resolve the confusion in the treatment package. I am happy to say, we have reached a mutual resolution. Although all of our treatment packages are time based, we feel that there was not a sufficient
explanation of the treatment guidelines when he began. We are extending Mr*** another three months to utilize his treatment plan. If there are any questions, please feel free to contact me.Sincerely,Michael G*** ***

We have sent in requests for more information on what contraindication is causing Mr*** to be unable to finish the treatment he signed up for. To date we have not received any information

I was able to speak with Mr*** last night when he returned from his Bahamas trip We have agreed to refund back the amount of $back to Mr*** in the form of a Cashier's check from my accounting office We have sent him our standard Release of Claims form, and are just
waiting for a signed copy to be returned Once received I will request his full refund which he should receive in to days. MrD*** is correct that he purchased a month treatment program from out clinic. His treatment began on January 22, and EXPIRED April 22, 2015. During the course of treatment MrD*** had open opportunity to come in and see our physician, request medications specially
compounded just for him. In regards to MrD***'s blatant lie about his medical doctor advising him not to take the erectile dysfunction medication I have included the email sent to us by MrD*** dated 4/21/2015. It is NOT a note from his physician, nor is it a note from any physician, it is merely a newsletter that states " Changes to Viagra and Levitra waiting time - Both of these medications previously required a hour waiting time before flying. The FAA recently changed this to an hour wait before flying for Viagra and hour wait before flying for Levitra". So even by his own omission (the newsletter he sent us) he is able to take the medication available during his treatment. When I spoke to MrD*** I reminded him that he only purchased a month block of time for his treatment and for him to call on 9/23/to ask for a refund when his treatment ended on 04/22/was highly unlikely. I explained to the patient that I would research his claims of previous calls to the clinic and would work out a partial refund for him based on the amount of time he had left if said calls were able to be proven. I found in my research of his file that he did in fact email us the newsletter (1) day prior to the end of his treatment and he was requesting a refund. I offered to refund him the amount for that (1) day that he had left in his program. . MrD*** argued that he did not have time to use our program as he is a pilot with a local company, I am very aware of the company he works with and they have a 7day on, day off policy. Again, I reminded MrD*** that he did not purchase a physical product from us but a block of time. MrD*** refused and hung up in an abrupt manner
