Complaint: ***I am rejecting this response because:
They never had me sign a credit card applicationThe only thing that they had me sign was new patient information like address, name, medical history and insurance informationThey wanted me to do everything electronicly but I ran out of time so I filled everything in the office but I'm sure they uploaded everything in their systemAfter the treatment they performed they had me counsel a financial officer and the first thing she asked for was two forms of ID which I thought was strange but I compliedAfter that she spoke to me about the credit card, I declined, and I never signed any document but she said I was preapproved for dollars but I declined the card againBut once again when I spoke to the financial officer who signed me up for the card I did not sign any documentation the only documents I signed were the new patient forms.Sincerely,*** ***
