This letter is to address the correspondence we received from your office dated March 18, 2015 regarding the above captioned matter.On February 4. 2015. Mr. [redacted] contacted our office with different insurances and spoke with our insurance specialist. J[redacted]. inquiring what insurance plans we are...

enrolled with. At this time, J[redacted] informed Mr. [redacted] that we are in network with a variety of plans. including a Cigna PPO plan. J[redacted] at no time encouraged or recommended Mr. [redacted] to enroll with this plan. She simply informed Mr. [redacted] that Cigna PPO was one of the plans we are currently in contract with and never went over procedures or ADA codes with him.On March I0. 2015. Mr. [redacted] was scheduled in our office for a comprehensive exam (D1110). a full mouth series of xrays (D0210) and an adult prophylaxis (D1110). This was recommended because we had never done a comprehensive exam on the patient. At this time Mr. [redacted] told the dental hygienist that he would like to refuse the radiographs. Mr. [redacted] signed and witnessed an “Informed Refusal Xray Consent Withheld” form. No dental xrays were taken at this appointment.Mr. [redacted] was charged the discounted contracted fees we have negotiated with Cigna PPO. (Please see the highlighted areas of the disclaimer information on Mr. [redacted]’s presented Membership Card.)Reviewing the attached description of D0150, Dr. C[redacted] did in fact perform a comprehensive exam which included reviewing his medical history and medications which took 5 to 10 minutes. We also did an oral cancer screening, evaluated each tooth and evaluated his hard and soil tissues. Patient was told of the limitations of the exam without intra-oral radiographs. We also asked him ifhe had any questions. At that time Mr. [redacted] made a comment about the news and Dr. C[redacted] very nicely stayed with Mr. [redacted] for a few minutes and had a nice discussion with him.Mr. [redacted] received a prophy and exam and services were discounted appropriately. The patient paid that day and had no concerns. Later that week, he called with concerns about the money he owed for his office visit and he did not return our call and tiled a complaint with the Revdex.com. Dr. C[redacted] called him personally to address his concerns. It is also important to note that the Comfort Dental Group has had excellent communication and exceptional customer servicehas been given to both Mr. [redacted] and his wife.We value our patients very much and take great pride in our work and hope this correspondence addresses Mr. [redacted]’s complaints in an understandable manner.Kind Regards,The Comfort Dental Group[redacted]Enclosures
