I love this place and I will trained anywhere else! They are very professional at what they do and will not leave unless you as the trainee is very happy at the end of the day...awesome ! Perfect 10! I have training there since the last 2 years and got more people in there to trained because they are just that good. They current and former law enforcement guys with a ton of knowledge to share with others to make our lives safer! You can't ask for a better training facility.

I will not train anywhere else but here!

Review: Completed a CPR class September 30, 2015. The 4 of us in class were told we would get our CPR certification cards and never received the cards. Multiple people have called recently to check on the status and have been given the run around. It has been 2 months and still no card.Desired Settlement: To actually receive the cards that we deserve.

Please take cognizance of the reasons why I withdrew from your SWAT Medic course on 29 April 2016.

You have constituted a training environment that is non-factual, unsafe, and not following accepted health and medical standards in American to say the least. Additionally, your training environment is abusive and discriminatory.

Its hard to believe any of your claims regarding your expertise in the medical field as many of the practices that AHA and American [redacted] (I am ACLS certified by AHA and an instructor with American [redacted]) recommends are contradicted by you without fact or merit. Please not AHA and American [redacted] operate their training as per ECC standards which is considered the gold standard for evidence based medicine, followed by almost all health and safety emergency responders and definitive care institutions in America.

Your statement that CPR in Civilian Trauma yields no survivors, in your slide 252, is based on a population of 138 patients! Note that for evidence based medicine,138 patients are not a sizable population that will yield conclusive findings nor does that number yield any statistically significant confidence intervals. Basic knowledge of math and statistics would have prevented you from making such training materials.

Application of CPR in civilian settings must be based on expert studies from American Heart Association's Emergency Cardiovascular Care (ECC) or the American [redacted] - to claim that these agencies are spreading false information for money - is scandalous, defamatory and libel.

I withdrew from your training due to the above issues and I am filing disputes for recovering the fee I paid for your services. I also want my registration for the May 28-29, 2016 TCCC training cancelled. I have no interest in pursuing any training with CCJA hereon.Desired Settlement: I have paid over $1000 for two trainings with CCJA. I want that entire fee to be refunded.

This statement is made to refute the statements made by [redacted] in regards to the above referenced Revdex.com case. I will address the major claim of your statement which will give understanding to the lack of substance to the claims: The power point and handouts that were used in class were developed by The “NAEMT “National Association of Emergency Medical Technicians. The course material is from the “LEFR-TCC” LAW Enforcement First Responder Tactical Casualty Care course. Derived from the Committee for “TCCC’ Tactical Combat Casualty Care. They were not created [redacted] In reference to Page # 91 in the student Guide (Slide 252 on the power point which is depicted on that page). And as I told you in class when you asked: Here is the study where the information on that slide came from: Rosemurgy et al. J Trauma 1993 The Journal of Emergency Medicine PREHOSPITAL TRAUMATIC CARDIAC ARREST: THE COST OF FUTILITY. [redacted] (C) [redacted] Of 12,462 trauma patients cared for by prehospital services from October 1, 1989 to March 31,1991, 138 patients underwent CPR at the scene or during transport because of the absence of blood pressure, pulse, and respiration. Ninety-six (70%) suffered blunt trauma, 42 (30%) suffered penetrating trauma. Sixty (43%) were transported by air utilizing county-wide transport protocols. None of the patients survived. Aggregate care cost $871,186.00. In 11 cases (8%), tissue for transplantation was procured (only corneas). Conclusion: Trauma patients who require CPR at the scene or in transport die. Infrequent organ procurement does not seem to justify the cost (primarily borne by hospitals), consumption of resources, and exposure of health care providers to occupational health hazards. The wisdom of transporting trauma victims suffering cardiopulmonary arrest at the scene or during transport must be questioned. Allocation of resources to these patients is not an insular medical issue, but a broad concern for our society, and society should decide if the "cost of futility" is excessive. As you can see, the study was statistically relevant and significant. If you would have spent more time in class listening to what was taught and less time on your phone talking to other people while interrupting my class, (and causing a distraction to the other students) you may have a better understanding of what was taught in class. I believe that one of the contributing factors was the language barrier; you had indicated that you are a non-native English speaker. As an instructor I know Language is a very complex thing, and communication between people speaking different languages is difficult. Language is a way of looking at the world, and even skilled translators can find it tricky to convey complex emotions and concepts, which can lead to misunderstandings. When you think about how often you misunderstand someone speaking your language, you can imagine how hard it is to get the full meaning from something a person with a different cultural background is saying to you. More importantly this was not a civilian class it was a TACTICAL SWAT medical course. The six other students in class were clearly aware of the difference. Your last statement regarding that I asserted other agencies are spreading false information for monetary gain is again attributed to lack of understanding of the subject matter at hand and the context. For example I indicated in class that in first aid class students in other schools are taught to elevate the legs when a patient is in shock. To which you agreed: I am sure this is what you instruct as a First Aid instructor for the [redacted]. Please see the below paper written by: [redacted] Curriculum Director [redacted], April 2013 With the rise of violence and injuries in our area lately, suppose you come upon the victim of a gunshot wound or a bad car accident. After you dial 911 (or for my emergency services followers, you arrive on the scene), what do you do for this person that is bleeding out? Most people would say, as is seen on TV and commonly taught among first aid courses, to lift their legs high in the air. The reasoning behind this is to get the blood from the legs to the torso, and increase the blood pressure to help get more blood to let the vital organs perfuse better – a method and position called Trendelenburg Position (Trendelenburg). Trendelenburg position for patients suffering from acute hypotension or hypovolemia (low blood pressure / low blood volume – colloquially referred to as shock) has been common practice for hospital and EMS professionals since the dawn of the first ambulance. Initiated by a German surgeon during World War I (Friedrich Trendelenburg), Trendelenburg was promoted as the premier, immediate intervention for shock for combat-wounded soldiers by lying them flat and raising the legs at least twelve inches above the torso; subsequently, the practice quickly gained widespread use and is now frequently performed – one study showed that 99% of critical-care nurses have performed this maneuver at least once for a patient in shock (1). But are you really helping the person by doing this? An exceeding amount of medical literature has come about stating that Trendelenburg position has no medical benefit to a patient in shock and may in fact cause further harm to a patient. One study found that, even though the blood pressure actually increased, there was no improvement of cardiac output (the amount of blood pumped out of the heart per minute) – meaning, even though the blood pressure went up, the physical amount of blood coming out of the heart did not change, resulting in no change in tissue oxygenation (the vital organs were not getting the help they needed) (2, 3). So really – what’s so bad about that, we want the blood pressure to go up and it went up? Not so fast – even though the goal is to bring the blood pressure up, the reasoning behind that is to have an increase in the amount of blood being pumped out of the heart (cardiac output) so a higher amount of oxygen can be delivered to the vital organs and prevent or deter any tissue damage (causing a heart attack, stroke, etc). Furthermore, even though the blood pressure may increase, the actual amount of blood found to be displaced from the legs to the torso was 1.8% (1). This means that for the average person (who normally has 5000mL of blood circulating at a given time), only 90mL was being added to the torso to treat their low blood pressure (about ¼ of an average sized soda can); to put this in perspective, the average amount of blood loss required for a person to be in shock is 2000mL – so ultimately, you are trying to fight a house fire with a dollar-store squirt gun. The real danger falls upon the critically ill with comorbidities (someone who is really sick, with multiple amounts of other diseases as well – such as heart failure, COPD, high blood pressure) – these are patients who are most likely to be in situations where their blood pressure will be low, and a healthcare practitioner or layman will treat them first by using Trendelenburg position. A study focusing on critically ill patients suffering from shock with multiple comorbidities found that there was a decrease in cardiac output, excessive weight on the diaphragm (the muscle that helps us breathe) from the weight of the intestines crushing up on it, and decreased lung volume – their heart weakened, the amount of blood they were perfusing decreased, and their ability to breathe effectively was compromised. On top of all of that, patients who were continuously placed in Trendelenburg suffered from increased cranial pressure (pressure on their head and brain) with subsequent retinal detachment (the eye separates from the membranes that help support sight) and paralysis of vital nerves from the increased pressure in the brain. So why is Trendelenburg still taught as common practice and still so actively used? If you didn’t pick up from my last article, change in medicine takes a long while – specifically in EMS and for emergency services. Newer textbooks and literature strongly advocate against its use; but word of mouth and experiencebased stories still account for a high amount of Trendelenburg’s persistence and perseverance. Ultimately, if a patient presents with hypotension – think twice before slamming their head down and raising their feet up to high heaven. You note that I claim that theses agencies are spreading false information for money…. “And that is scandalous defamatory and libel” in your words. You take that statement out of context. Ie: if anyone were to continue to teach information that is out of date and inaccurate and get paid for, I feel it that is scandalous. I will be more than happy to give you a full refund of all your money.
