January 26, 2017We received your letter regarding [redacted] and would like to address her concerns[redacted] came to our office and received a free consultation on 7/6/for our Vanquish systemThe Vanquish system uses a non invasive radio frequency that assists body in dissolving fat On 7/6/we also did a free consultation for nutrition at this timeWe encourage moderate exercise and proper nutrition to every patient using Vanquish for maximum resultsOn 7/7/ [redacted] signed consent and paid for sessionsAt the time of her first appointment we also did weight and measurementsThe patient consent (line#2) states that results vary from person to personWe have attached a copy of the consent for your review.8/10/was session of treatments, [redacted] stated she was not seeing desired resultsWe spoke about diet, exercise and that Vanquish will continue to expel fat up to weeks after initial treatmentWe informed her that we always do a re-measure at that mark so you can see your resultsWe also reminded her that results take time and are different for all patientsShe then inquired about cosmetic injections on the faceShe requested that we trade her remaining Vanquish treatments for cosmetic injectionsDrK [redacted] informed her that we are unable to do that.On 9/1/Session of sessions we measured [redacted] and she had gained 1"We attempted to review nutrition and exercise with [redacted] We also reminded her that the system continues to dissolve fat cells after the 8th sessionDrK [redacted] also explained that at the time of weeks measurement if desired results are not achieved we would provide complimentary treatments until she loses 2" [redacted] was unsatisfied with this offer and again inquired about trading the remaining sessions for cosmetic injectionsWe informed her again that the two procedures are completely separate and individual procedures and could not be exchanged.On 9/8/ [redacted] had her 7th session scheduled she did not come at the designated appointment timeHowever, she called later and requested a copy of her sign consent formWe copied the form and she came to our office to pick it up.Comprehensive Family Practice takes great pride and commitment in the care and Satisfaction of all our patientsWe have honored the consent signed by [redacted] and offered additional complimentary sessions to ensure her personal goals are achieved.Thank you for your time and attention in this matter.Sincerely,Comprehensive Family Practice staff

March 22, 2017I am replying to the last letter I received from the Revdex.com regarding [redacted] , this letter is in addition to my original reply on January 2017.As stated in my initial reply Comprehensive Family Practice takes great pride and commitment in the care and satisfaction of all our patientsI did offer [redacted] additional sessions to reach her goalI did have a technologist present in the room during this discussion on 9/1/However, I am aware that there is no definitive way to document this conversation.Each time a patient has an appointment for Vanquish we take a weight and water measurementWe take measurements of the abdomen in accordance with the protocol associated with the machineWe document everything as part of the patient's treatment recordsWe request the patient initial each time a treatment is givenI can assure that all the records are in order and match her scheduled appointmentsI have enclosed a copy of her treatment record with this reply.During a consultation we discuss all options available at our office for weigh lossWe never "try to talk patients into purchasing diet plans." It is routine to discuss the options like Vanquish, nutritional consultations and nutritional programs such as [redacted] [redacted] came to us with the intent to lose inches and manage her weightA package was designed specifically to her needs [redacted] was at her 6" session with us when she gained 1" we attempted to discuss her nutrition and lifestyle planning with her [redacted] was reminded that we have a certified nutritionist on staff and access to her is

I am rejecting this response because:They had me stand on a scale...and DO NOT tell me that they said that they would continue treatments until I lost inches because that is a flat out lie! They also did not have me sign that attendance form until my 4th visit! she had me sign all treatment dates in one dayI have contacted the Attorney General regarding this matterI was often left alone in the treatment room and the lights would get so hot that I had to cover up in order of fear of getting burnedI had told the assistant this and she just moved the light an inchI did not finish my treatments, and was talked into treatments after I expressed how it wasn't working for meI did not meet the doctor until I starting expressing how it wasn't workingI have never seen or known about the body chart, so I have no idea when that was done or if it is even accurateI have been and will continue to dispute this until it is resolvedI will also go to social media if this is not resolved soonI am a single, hardworking person who was just trying to feel better about myself and I trusted this Practice to do thatI was wrong and I feel I am owed my money back for services not rendered and the very unprofessional matter in which I was treated

On 7/6/17 we also did a free consultation for nutrition at this time. We encourage moderate exercise and proper nutrition to every patient using Vanquish for maximum results. On 7/7/16 [redacted] signed consent and paid for 8 sessions. At the time of her first appointment we also did weight and measurements. The patient consent (line#2) states that results vary from person to person. We have attached a copy of the consent for your review.8/10/16 was session 4 of 8 treatments, [redacted] stated she was not seeing desired results. We spoke about diet, exercise and that Vanquish will continue to expel fat up to 12 weeks after initial treatment. We informed her that we always do a re-measure at that mark so you can see your results. We also reminded her that results take time and are different for all patients. She then inquired about cosmetic injections on the face. She requested that we trade her remaining Vanquish treatments for cosmetic injections. Dr. K[redacted] informed her that we are unable to do that.On 9/1/16 Session 6 of 8 sessions we measured [redacted] and she had gained 1". We attempted to review nutrition and exercise with [redacted]. We also reminded her that the system continues to dissolve fat cells after the 8th session.
