Page of 4, the incident reportThere’s some numbers CFS ***It says *** *** reported his PCA, first name Dee stole from his freezerShe definitely stole meat from my freezerThey wouldn’t tell me her real name at first because they didn’t give me a background checkAnd they’re in on the court views as being involved in the courtThe one, the Rhonda *** J***, she’s got different complaints against her that she had to go to court forShe’s a chronic Kleptomaniac is the only thing I can figureWhich is true, this is true, she stole meat from my freezerMy neighbor had just went up to *** he bought me a hundred dollars’ worth of beef, hundreds of dollars of pork and other things from the storeWe brought it down here and she was here when I was cutting it up; in fact she even helped me put it awayWell, what she was doing is putting it in her big gunny sack out there and taking it out to her rigIt never hit the freezer‘Cause there wasn’t anything in it in the morning and her and I was the only one in the houseSo what would you think? You know she took itAnd then it says, uh, “He has already fired her” and he said “He did not see her take the meat, but noticed the freezer was less full than it had been” Well, the one was empty, just aboutAnd then when I told her not to come to work in the morning, she said “Oh, what did I do?” and I said “you know what you did” I didn’t want to tell her about stealing the meat, I didn’t want her to hide the meatI thought the police would go check on it, and I didn’t want to spook herSo what I did was just say “You know what you did” and she said “Oh, that.” And that’s all she said about it, and then I hung up She knows what she did, that’s for sureShe did it, that’s allThe trooper did come, he told me finally, after phone calls, he says “We don’t’ have the manpower or the time to check these petty thefts” he says “they’re getting away with itI know you think it’s terrible but we just can’t do it.” And he said, “ All I can do is advise you to lock things up.” Well, I got locks on the freezer, I got locks everywhere nowThat’s what I had to do, is just buy locks, even had to pay a carpenter to do a lot of it, because I couldn’t do it
And then they put here, of all the *** things they put here, it said “the woman declined Mr*** advances, she received a call saying she could not come back there and he said ‘You know why.’ *** confirmed using his wording when he told her not to return.” Well, I didn’t tell her that she’s, I didn’t want to accuse her of stealing the meat right away, because that would’ve maybe messed things up for the state police if they went over thereI thought that’s what they do, but they didn’t do itNobody did nothing
Everything is just about in the same lightThe sentences are wrong, what they saidThe PC’s of course, are chronic liars and thieves, and the two guys that are running the show, that make the money off of them, they’re lying right along with them instead of owning up to itThey’re just telling all kinds of lies and trying to get out of it
The essence of the whole verbatim is the fact that it carries that sort of attitude and directionWhich is totally wrong, and I’d swear to it in court and I want to get them in court really bad

Ms***;We filed the report that was missing with the State of Missouri, following our correspondence regarding a past due tax bill This should have taken care of any taxes that Missouri should have shown owed under our time as your agent Often times when clients move from one agent to another, switching tax accounts can become tricky Our intention was not to “blame” you, however the address on the tax notice belongs to PCG not Consumer Direct We did not receive knowledge of a tax due until February 16th So PCG received the tax notice, not Consumer Direct PCG did send this on to us, but did not provide any tax notices. On February 17, 2017, I had MsM*** ask you to provide a copy of the supporting documentation for the taxes due and payment made so that we could ensure that we would not make a duplicate payment to the State of Missouri We did receive information regarding the tax bill at that time At that time we had asked for a voided check so that we could refund the money and get it into your account as soon as possible We have not received the voided check, so at this time I will as are accounts payable department to send you a check for $and mail it to *** *** *** *** *** ** ***If this address is not correct, please let me know. Please confirm that this is where the check needs to be mailed I have also attached all emails and documents received to date as reference. Please let me know if you need anything else.Sincerely; Katherine T*** | Payroll Manager Consumer Direct Care Network | Payroll

Ms***;We filed the report that was missing with the State of Missouri, following our correspondence regarding a past due tax bill? This should have taken care of any taxes that Missouri should have shown owed under our time as your agent? Often times when clients move from one agent to another, switching tax accounts can become tricky? Our intention was not to “blame” you, however the address on the tax notice belongs to PCG ? not Consumer Direct? We did not receive knowledge of a tax due until February 16th? So PCG received the tax notice, not Consumer Direct? PCG did send this on to us, but did not provide any tax notices. On February 17, 2017, I had MsM*** ask you to provide a copy of the supporting documentation for the taxes due and payment made so that we could ensure that we would not make a duplicate payment to the State of Missouri? We did receive information regarding the tax bill at that time? At that time we had asked for a voided check so that we could refund the money and get it into your account as soon as possible? We have not received the voided check, so at this time I will as are accounts payable department to send you a check for $and mail it to *** *** *** *** ? *** ** ***If this address is not correct, please let me know. Please confirm that this is where the check needs to be mailed? I have also attached all emails and documents received to date as reference. Please let me know if you need anything else.Sincerely; ? Katherine T*** | Payroll Manager? Subject: Revdex.com complaint ID [redacted] Dear [redacted], Thank you for sending us the complaint ID [redacted] and allowing us the opportunity to respond to provide our side of the events listed in the complaint. Please respect that we want to respond to Mr. [redacted] complaint but in no way...

want to jeopardize his HIPAA rights under the Personal Care Assistance (PCA) program. Please treat all of the documents that we are sending you with the utmost confidentiality and know that we are only submitting them to relay the details surrounding Mr. [redacted] complaint and that we did not act inappropriately in our role as an agency. Mr. [redacted] was referred to our agency on 2/26/15 by the Independent Living Center so we could assist Mr. [redacted] in seeing if he could receive PCA services. PCA services are designed to assist the elderly and Alaskans with disabilities with activities of daily living (ADL's} so they can remain safe and independent in their home and community. Mr. [redacted] went through the process to work with the State of Alaska and the Medicaid program to be assessed and receive a PCA service plan of the tasks that he would need assistance with in his home. Mr. [redacted] chose to work with a consumer directed model which means he is the managing employer i.e. interviews, hires, trains, supervises, and can terminate the personal care assistants (PCA's} that assist him in providing the ADL's that he requires. Our agency is the employer of record and fiscal intermediary and we act as the liaison between the State of Alaska and the consumer, Mr. [redacted] in this case. We handle the processing of the timesheets for the PCA's, workman's comp, taxes, and to make sure that the consumers and PCA's are following the rules and regulations of the PCA program. Mr. [redacted] PCA services started with our agency on 3/31/15. On 4/16/15 Mr. [redacted] called our Program Coordinator Collins F[redacted] and asked him to come to his house as he felt that his PCA had stolen several items from his freezer. The issue was investigated by Collins and his supervisor Jim T[redacted], the Kenai Peninsula Regional Coordinator, who suggested that Mr. [redacted] contact the State Troopers and file a report. At that time Mr. [redacted] dismissed the PCA and on 7/1/15 he hired another PCA to assist him with his ADL's. On 7/24/15 Mr. [redacted] again contacted Collins F[redacted] to report that his new PCA had also stolen many items from him. Mr. [redacted] at that time got upset at our agency and claimed that we were a "bunch of crooks". Mr. [redacted] wanted to know if background checks were completed on the PCA's and accused the current PCA of having several felonies. Our office explained to Mr. [redacted] that we follow the regulations closely and all PCA's undergo a background prior to employment and are not eligible to work until a "provisional" clearance and after that a final clearance that is good for six years.is received from the State of Alaska Background Check Program. On 7/28/15 Consumer Direct sent Mr. [redacted] a 30 day notice that we were discharging him from our agency and letting him know that he could transfer his services to another agency of his choice. On 8/6/15 Mr. [redacted] transferred his PCA services to [redacted]. On 8/21/15 Consumer Direct received a complaint from the State of Alaska Attorney General's office with a similar complaint that Mr. [redacted] had filed with your agency. Our agency at that time submitted a detailed timeline of events surrounding Mr. [redacted] actions with our agency (see attached). We also reached out to the State Troopers to get a copy of the police reports (also attached). As you can see from the initial police report that Mr. [redacted] claim was unfounded. We believe as an agency we acted appropriately by investigating the matter and interviewing both Mr. [redacted] and the PCA's in both cases as indicated in Attachment 1. Both times we recommended to Mr. [redacted] to contact the State Troopers to file a report as shown in Attachment 2. As mentioned above Mr. [redacted] is the managing employer and was in charge of interviewing and hiring his PCA's. Please don't hesitate to call if you have any further questions or concerns regarding our response.
